Dear Mr. Riper:
The New York State Department of Health Glens Falls District Office has reviewed and
approved Engineering pians to install a replacement OWTS for one 3-bedroom home located at
Lot# 33 Briwood Circle in the Forest Park Mobile Home Court, located in the Town of
Queensbury, Warren County, NY.
A new 1250-gallon concrete septic tank with an effluent filter will flow to a new concrete
distribution box with speed levelers. The absorption field consists of two 55-feet long ADS
RANCOR ARC 24 Chambers. Gravelless Chambers such as these allow a 25% reduction in
total absorption trench length over a conventional lateral design. The proposed design provides
a total length of laterals that exceed the requirements of Appendix 75-A (2010).
The plans are approved with the following conditions:
1. THAT the Engineer obtains required Permits from the Town of Queensbury and
Warren County, and,
2. THAT the Engineer certifies in writing that the system is installed in conformance
with the plans approved this day, and,
3. THAT, when completed, the engineer provides as-built drawings showing any
accepted changes to the plans, and, any topographical anomalies and/or previously
undocumented underground artifacts affecting the installation.
Two sets of stamped plans are being provided, one to the Park owner and one to the Town's
Building Code office, and one set is being retained for our records.
The purpose of the Department of Health review is to verify compliance with the Design
Handbook (2012) and the Public Health Law, Appendix 75-A (2010) and in no way guarantees
