Dear Dave:
On June 23, 2016, we inspected the existing absorption system located at 125 Mannis Road and
found that the existing absorption system was in failure. We measured the area available and
checked the surrounding properties for wells. We found that the existing lawn area where the
original absorption field is Iocated could accommodate a 20 ft wide by 35 ft long replacement
absorption bed. Both parcels on either side of the proposed replacement system do not have wells
within the 100 ft separation requirement. The Hensler / Hughes parcel uses lake water as its source
of potable water.
The deep soil test pit in the approved plans where as follows:

Dry @ 70"
The design flow for this 6 bedroom residence at 110 gpd per bedroom (660 gpd design flow),
requires 695 sf of absorption bed. The replacement absorption system should be constructed as a
standard bed system. The absorption system will consist of the existing 1,000 gallon and 1,250
gallon septic tanks, a new distribution box and 20 ft wide by 35 ft long absorption bed with 4 — 30
foot long absorption laterals located to the south of the existing residence.
We recommend that during removal of the existing absorption field that the engineer perform a
deep hole test pit to confirm that the stony, dark gray fill & wood had been removed during the
original installation. If this soil is found, we have advised the client and the installer that the
material would have to be removed and replaced with loamy fill. We will perform a percolation test
and include the results in the septic system as-built documents.

This letter is to inform you that we inspected the completed replacement septic system for the
Hensler/Hughes residence located at 125 Mannis Road on July 5,2016.
The replacement septic system as installed was for a six bedroom house and consisted of the
existing 1,000 and 1,250 gallon septic tanks and a 20 ft wide by 35 ft long absorption bed with 4 —
30 ft long interconnected absorption laterals constructed with stone and perforated pipe. The
system conforms to the requirements of the approved replacement design.
