This letter is to inform you that the pumped dosed Residential Bed Septic System at the
above referenced location was installed in accordance with the standard plan
specifications as submitted by my office, to include sheets ST-001 dated 03/20/12,Rev 1,
W-001 dated 03/20/12, Rev 2, and W-002 dated 03/20/12. Rev 2.
The Site Plan details the location of the as built field on the property. Required setbacks
to property lines, structures and wells have been maintained.
The new system has been constructed in the location indicated and in the manner detailed
in the approved plans and specifications.
