Mr. O'Brien,
GMS has not performed a site visit to the Bank of America branch located at 183 Quaker Road in
Queensbury,NY to inspect the repair work at the drive thru canopy firsthand. However,GMS has
reviewed the special inspection reports prepared by Gifford Engineering dated 9/10/12,the concrete
compression tests prepared by Construction Technology dated 9/5/2012,and the photos taken by the
contractor during construction. The drive thru appears to have been repaired in accordance with the latest
set of drawings prepared by GMS dated 9/12/2012"Issue for Record."
