The design plans reviewed and approved by the Town as associated with the wastewater system application are as follows: Proposed Wastewater System Replacement for Lawrence.1. & Christine B. Busino Residence 65 Mason Road Town of Queensbury Tax Parcel 226.12-1-14 • Sheet 1 of 2 Site Plan with Proposed Wastewater System issuedl0-16-13 • Sheet 2 of 2 Design Notes and Details issued 10-16-13 During the course of construction I was on the job site to observe key components of the system installation culminating with the `start-up' of the pump assembly and control panel system on May 6, 2014. Field measurements of the d-box and absorption system corners are consistent with the design plan so no `as-built' drawing is otherwise provided. Based on my inspection of the system start-up it is my opinion that the wastewater system is in compliance with the intent of the approved design plan and is ready for operation.
