Dear Mr. O'Brien:
This office prepared initial design details for septic system work on the above-referenced parcel
in 2005. The current owners, William and April Chase, revised the location of proposed
infrastructure from the original suggested layout in order to meet their needs.
I conducted a soils test pit in the area of the proposed septic system on November 4, 2014
which confirmed that the shallow absorption trench system was appropriate, as per the original
design. I inspected the installation of the absorption trenches, distribution box, and septic tank
on December 23, 2014 and subsequently inspected final installation on April 21, 2015. The
system, as installed, consists of a 1,000-gallon pre-cast concrete septic tank, a concrete
distribution box, and three 55-foot-long pipe-in-stone conventional absorption trenches installed
in shallow configuration. Attached is a mark-up of the actual surveyed building, tank, and field
locations. Record drawing dimensions have been added.
It is my opinion that the septic system is installed in general conformance with the original
design details and the attached record drawing mark-up. Please contact me if you have any
