Our office prepared a wastewater design for Ronald and Cynthia Mackowiak at the above-
referenced property. The system consists of an E-One grinder pump station, a 1,250-gallon
precast concrete septic tank, a 1,000-gallon effluent pump station, and an absorption field
consisting of five 22-inch-wide open-bottom gravelless chambers at 28 feet each.
A representative from our office visited the site during system installation. Based upon our site
inspection and our discussions with the installer, it is my opinion that the system was installed in
general conformance with the approved plan and the attached record drawing.
