accordance with our Service Supplier Proposal dated February 7, 2016, and McDonald's purchase
Order No. 1277214. We understand that this survey was requested due to planned demolition
activities of the above-referenced building.
Asbestos was identified in the samples collected and analyzed for this project. Please refer to the

EXECUTIVE SUMMARY
Terracon Consultants, Inc. (Terracon) conducted an asbestos surrey of the McDonald's located
at 819 Route 9 in Queensbury, New York. We understand this asbestos survey was requested
due to the planned demolition of the building. The purpose of this survey was to sample and
identify suspect asbestos-containing materials (ACM) and provide information regarding the
identity, location, condition, and approximate quantities of ACM in interior and exterior building
components. The survey was performed on February 16, 2016 by an Asbestos Hazard
Emergency Response Act (AHERA) accredited and State of New York certified asbestos
inspector in general accordance with our Service Supplier Proposal dated February 7, 2016,
McDonald's Purchase Order No. 1277214, and the sampling protocols established in United
States Environmental Protection Agency (USEPA) 40 Code of Federal Regulations (CFR) Part
763, Subpart E, known as the AHERA. Terracon collected 102 bulk samples from 34
homogeneous areas of suspect ACM.

The restaurant was occupied and operating at the time of the survey. As such, certain building
areas, systems, structural components, or surfaces could not be observed because the space or
surface was unsafe or impractical to access, disassemble, or remove, and thus, were not
surveyed. The inaccessible areas included the following:
The electrical systems in the building were energized at the time of the survey and
therefore were not sampled. The vinyl coating, internal paper and fibrous
insulation surrounding the wires are considered non-friable suspect materials and
must be assumed to be ACM unless sampling determines otherwise.

Subsurface damp-proofing on exterior foundation walls (if present);
Freezer/cooler panels;
Mansard roof interior (where roof system contacts walls) and soffit.
The materials listed above must either be assumed to contain asbestos or Terracon must return
to the site when safe access is available to conduct destructive sampling prior to demolition
activities.

Because the proposed demolition activities may crush or pulverize AGM, 7erracon recommends
they be removed before any demolition begins by a qualified, licensed asbestos abatement
contractor in accordance with a project design prepared by a licensed asbestos project designer.
Licensed asbestos abatement contractors should be contacted to obtain competitive bids for
removal of the ACM. At your request, 7erracon can provide a proposal for developing asbestos
abatement specifications (project design) and for performing abatement observation and air
monitoring.
Additionally,the following suspect material was identified and sampled during the sun/ey. Analytical
results, confirmed by analysis, indicate the following material does not contain asbestos in
concentrations greater than 1% and is therefore not an EWA-regulated material. This material
should be brought to the attention of contractors who may disturb the material so that they can
comply with the Occupational Health and Safety Administration's (OSHA) asbestos provisions
found in 29 CFR 1926.1101:

1.0 INTRODUCTION
Terracon Consultants, Inc. (Terracon) conducted an asbestos survey of the McDonalds' located
at 819 Route 9 in Queensbury, New York. The survey was conducted on February 16, 2016 by
an Asbestos Hazard Emergency Response Act (AHERA) accredited and state of New York
certified asbestos inspector in general accordance with our Service Supplier Proposal dated
February 7, 2016, and McDonald's Purchase Order No. 1277214. The interior and exterior
building components were surveyed, and homogeneous areas of suspect asbestos-containing
materials (ACM)were visually identified and documented. Although reasonable effort was made
to survey accessible suspect materials, additional suspect but un-sampled materials could be
located in wails, in voids or in other concealed areas. Suspect ACM samples were collected in
general accordance with the sampling protocols outlined in United States Environmental
Protection Agency (USEPA) 40 Code of Federal Regulations (CFR) Part 763 Subpart E 763,
known as the AHERA.
Samples were delivered to an accredited laboratory for analysis by Polarized Light Microscopy
(PLM) via New York State Method 198.1 (friable materials) and 198.6 (non-friable organically
bound or NOB materials). If PLM NOB analysis resulted in an inconclusive result, or if "trace"
amounts of asbestos (less than 1 percent) are detected within a sample, the laboratory was
instructed to use Transmission Electron Microscopy (TEM) analysis per New York State 198.4
Method, for confirmation.
1.1 Project Objective
We understand this asbestos survey was requested due to the planned demolition of the on-site
building to satisfy requirements of the USEPA 40 CFR Part 61, Subpart M, the National Emission
Standards for Hazardous Air Pollutants (NESHAP).
1.2 Reliance
This report is for the exclusive use of McDonald's USA, LLC for the project being discussed.
Reliance by any other party on this report is prohibited without written authorization of Terracon
and McDonald's USA, LLC. Reliance on this report by McDonald's USA, LLC and all authorized
parties will be subject to the terms, conditions, and limitations stated in the proposal, this report,

Terracon's MSA is the aggregate limit of Terracon's liability to McDonald's Corporation this report
and Terracon's MSA.
2.0 BUILDING DESCRIPTIONS
The subject building is an approximately 4,600 square-foot McDonald's restaurant building. The
wood framed with a partial basement and has masonry exterior wails. The interior of the building
is finished with ceramic floor tiles, gypsum drywall, ceramic mail tiles (bathrooms and portions of
the kitchen), and wallpaper. The main roof is fat and constructed with rolled asphalt membrane,
a mop-coat, black fel paper, and a vapor barrier over a wood deck. An asphalt-shingled mansard
roof wraps around the perimeter of the building. The restaurant is heated with natural gas fired
forced hot air units mounted an the roof.
3.0 FIELD ACTIVITIES
The survey was conducted by Ms. Megan Cruz, an ANERA accredited and State of New York
certified asbestos inspector. A copy of Ms. Cruz's asbestos inspector certification is attached as
Appendix D. The survey was conducted in general accordance with the sample collection
protocols established in USEPA 40 CFR Part 763, Subpart E, Section 763.86, AHERA. A
summary of survey activities is provided below.
3.1 Visual Assessment
Survey activities were initiated with visual observation of the interior and exterior of the buildings
to identify homogeneous areas of suspect ACM. A homogeneous area (HA) consists of building
materials that appear similar throughout in terms of color and texture with consideration given to
the date of application. Interior assessment was conducted in visually accessible areas of the
building. Additionally, the roofing system was assessed.
Terracon lifted floor coverings in several areas, where possible, and did not observe additional
floor coverings/layers except where noted in this report; however, as Terracon could not assess
beneath all floor coverings in all areas, there may be isolated areas of additional suspect material
present beneath existing flooring.
3.2 Physical Assessment
A physical assessment of each homogeneous area of suspect ACM was conducted to assess the
friability and condition of the materials. A friable material is defined by the USEPA as a material
which can be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability
was assessed by physically touching suspect materials.

3.3 Sample Collection
Based on results of the visual observation,bulk samples of suspect ACM were collected in general
accordance with USEPA AHERA sampling protocols. Samples of suspect materials were
collected from randomly selected locations in each homogeneous area. Sulk samples were
collected using wet methods as applicable to reduce the potential for fiber release. Samples were
placed in sealable containers and labeled with unique sample numbers using an indelible marker.
The selection of sample locations and frequency of sampling were based on Terracon's
observations and the assumption that like materials in the same area are homogeneous in
content.
Terracon collected 102 bulk samples from 34 homogeneous areas of suspect ACM. A copy of the
laboratory report and chain of custody are included as Appendix B. Photographs of the sample
locations are attached in Appendix C.
3.4 Sample Analysis
Bulk samples were submitted under chain of custody to EMSL Analytical, Inc. of Piscataway, New
Jersey for analysis by PLM via New York State Method 198.1 (friable materials) and 198.6 (NOB
materials). If PLM NOB analysis resulted in an inconclusive result, or if "trace" amounts of
asbestos (less than 1 percent)are detected within a sample, the laboratory was instructed to use
Transmission Electron Microscopy (TEM) analysis per New York State 198.4 Method, for
confirmation. The percentage of asbestos, where applicable, was determined by microscopic
visual estimation. EMSL Analytical, Inc. is accredited under the National Voluntary Laboratory
Accreditation Program (NVLAP)Accreditation No. 101048-0.
The laboratory was instructed to analyze samples from each homogeneous area until the first
sample containing asbestos was identified (Positive Stop). The laboratory analytical data report
is attached in Appendix B.
4.0 REGULATORY OVERVIEW
The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and
asbestos waste disposal practices. The asbestos NESHAP regulation also requires the
identification and classification of existing ACM according to friability prior to demolition or
renovation activity. Friable ACM is a material containing more than 1% asbestos that, when dry,
can be crumbled, pulverized or reduced to powder by hand pressure.All friable ACM is considered
regulated asbestos-containing material (RACM).

The asbestos NESHAP regulation classifies ACM as either RACM, Category I non-friable AGM
or Category 11 non-friable ACM. RACM includes all friable ACM, along with Category I and
Category Il non-friable AGM that has become friable, will be or has been subjected to sanding,
grinding, cutting or abrading, or ACM that has a high probability of becoming or has become
crumbled, pulverized, or reduced to powder in the course of renovation or demolition activity.
Category I non-friable AGM are exclusively asbestos-containing packings, gaskets, resilient floor
coverings, resilient floor covering mastics and asphalt roofing products that contain more than i%
asbestos. Category If non-friable ACM are all other non-friable materials other than Category 1
non-friable ACM that contain more than I% asbestos.
Asbestos activities are regulated by the New York State Department of Labor (NYSDOL) under
the New York Regulations Industrial Code Rule 56 (12 NYCRR Part, 56), enforces the asbestos
NESHAP. The owner or operator must provide New York State Asbestos Control Bureau with
written notification at least 10 calendar days prior to the commencement of asbestos abatement
activities that will disturb RACM in amounts greater than or equal to 160 square feet or 260 linear
feet.
NYSDOL enforces the Occupational Safety and Health Administration (OSHA) standards at 29
CFR 1910.1001 and 29 CFR 1926.1101, which regulates employee exposure to asbestos. The
OSHA standard requires that employee exposure to airborne asbestos fibers be maintained at or
below 0.1 fibers per cubic centimeter of air (0.1 f/cc) as an eight hour time weighted average
(TUNA)and not exceed i.0 fibers per cubic centimeter of air(1.0 f/cc)over a 30 minute time period
known as an excursion limit (EL). The TWA and EL are known as OSHA's permissible exposure
limits (PELs). The OSHA standard classifies construction and maintenance activities which could
disturb ACM, and specifies work practices and precautions which employers must follow when
engaging in each class of regulated work.
5.0 FINDINGS AND RECOMMENDATIONS

6.0 LIMITATIONS/GENERAL COMMENTS
Terracon did not perform sampling which required demolition or destructive activities such as
dismantling of equipment or removal of protective coverings. Reasonable efforts to access
suspect materials within known areas of restricted access (e.g., crawl spaces) were made;
however, it should be noted that suspect materials,other than those identified during the February
3, 2016 survey may exist within the site building. Should suspect materials other than those which
were identified during the survey be uncovered prior to or during the abatement and demolition
process,those materials should be assumed asbestos-containing until sampling and analysis can
confirm or deny their asbestos content.
This asbestos survey was conducted in a manner consistent with the level of care and skill
ordinarily exercised by members of the profession currently practicing under similar conditions in
the same locale. The results,findings, conclusions and recornmendations expressed in this report
are based on conditions observed during our survey of the building.The information contained in
this report is relevant to the date on which this survey was performed, and should not be relied
upon to represent conditions at a later date. This report has been prepared on behalf of and
exclusively for use by McDonald's Corporation for specific application to their project as
discussed. This report is not a bidding document. Contractors or consultants reviewing this report
must draw their own conclusions regarding further investigation or remediation deemed
