RESOLUTION AUTHORIZING ENGAGEMENT OF C.T. MALE
ASSOCIATES FOR PROFESSIONAL DESIGN AND CONSTRUCTION
ADMINISTRATION ENGINEERING SERVICES RELATED TO CAREY
ROAD SANITARY SEWER DISTRICT AND ESTABLISHING
CAPITAL PROJECT FUND #221

WHEREAS, the Town of Queensbury's Wastewater Director requested proposals for
professional Design and Construction Administration Engineering Services (Services) related to
establishment of the Carey Road Sanitary Sewer District(Project), and
WHEREAS, the Wastewater Director and Purchasing Agent have received and reviewed three
(3) proposals, with the lowest submitted proposal submitted by C.T. Male Associates, and therefore
the Wastewater Director and Purchasing Agent have recommended that the Town Board
authorize engagement of C.T. Male Associates for such Services in the amount of $111,200 as
delineated in C.T. Male Associates' November 28, 2017 proposal presented at this meeting, and
WHEREAS, the Town Board accordingly wishes to establish a Capital Project Fund
related to the Project, and
WHEREAS, the Water Superintendent has advised the Town Board that there are times
when Change Orders may become necessary related to such Services and has requested that the
Town Board also authorize him to approve and sign certain Change Orders up to a 10%
contingency for Change Orders that he deems necessary or appropriate, and
WHEREAS, the Town Board wishes to authorize a withdrawal and expenditure in the total
estimated amount of$122,320 from Capital Reserve No. 64 toward this Project, and
WHEREAS, in accordance with New York State General Municipal Law §6(c), the Town
Board is authorized to withdraw and expend funds from Reserve 464 subject to permissive
referendum,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby authorizes and directs engagement of
C.T. Male Associates for professional Design and Construction Administration Engineering
Services (Services) related to the Carey Road Sanitary Sewer District for the amount of$111,200
as delineated in C.T. Male Associates' November 28, 2017 proposal as presented at this meeting,
and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs establishment of the
"Carey Road Sanitary Sewer District Capital Project Fund 4221," which Fund will establish
funding for expenses associated with the Project to be funded from the Town's Capital Reserve 464,
and
BE IT FURTHER,
RESOLVED, that the Town Board hereby finds that the withdrawal and expenditure for such
Project are expenditures for a specific capital project for which the Reserve Account was established,
and
BE IT FURTHER,

RESOLVED, that the Town Board hereby authorizes the Town Wastewater Director to
approve and sign Change Orders pertaining to such Services up to a 10% contingency or in the
total amount not exceeding $11,120 that he deems necessary or appropriate, and
BE IT FURTHER,
RESOLVED, that the Town Board hereby authorizes the Town Budget Officer to take all
actions necessary to amend the Town Budget, make any adjustments, budget amendments, transfers
and prepare any documentation to effectuate the transfer from Reserve No.: 64 in the amount of
$122,320 to fund such Project and establish initial appropriations and estimated revenues for
Capital Project Fund 4221 in the amount of$122,320, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Budget
Officer to establish the following accounts for such appropriations and revenues:
• Increase Appropriation 001-9950-9030-0064 Transfer to Capital Project - $122,320;
• Increase Revenue 221-0000-55031 Interfund Transfer - $122,320;
• Increase Appropriation 221-8120-2899 Capital Construction - $122,320; and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor to
execute any needed agreements and/or documentation, and the Town Supervisor, Wastewater
Director, Purchasing Agent and/or Town Budget Officer to take such other and further actions as may
be necessary to effectuate the terms of this Resolution, and
BE IT FURTHER,

RESOLVED, that this Resolution is subject to a permissive referendum in accordance with the
provisions of Town Law Article 7 and the Town Board hereby authorizes and directs the Town Clerk
to publish and post such notices and take such other actions as may be required by law.

Proposal for Professional Engineering Services
Dear Mr.Harrington:
The following represents our proposed scope and fee for professional engineering services
associated with the above referenced request for proposal.
Our understanding of the project is that three distinct areas within the Town and City of Glens
Falls will contain sewer improvements. These areas are as follows:
• Carey Rd Industrial Park Low Pressure Sewer. This will include design,bidding and
construction related services for the installation of approximately 12,200 if of 1.25"-4"
low pressure sewer and laterals. The layout will be consistent with the approved Map,
Plan and Report of July 2016.
• Broad St Gravity Sewer Replacement,Robertson to Hudson. This section will include
design only of a replacement of existing gravity sewer with a new 15" for a length of
760 if including laterals.
• Technical Park Sewer District Flow Measurement Device. This will include design,
bidding and construction phase services for a new flow meter to be located on the City
side of Luzerne Rd. Per the RFP, an alternative to a new flow meter would entail
retrofitting a flume in an existing manhole on the Town side.
As further described in the RFP,the project is broken down into two phases:

Phase I - Design and Bidding of each of the three sections described above. Specific tasks
anticipated are as follows:
1. Kickoff Meeting with Town Wastewater Department and City of Glens Falls.
2. Preliminary Design. Review with Town and City.
3. Cost Estimate of preliminary design elements.
4. Collect soils information via geoprobes for the Carey Rd sewer district area only.
Geoprobes will be conducted at approximate 500 foot spacing.
5. Complete topographic survey for the Broad St.sewer section only.
6. Complete mapping of each of the three areas at a scale of 1" =40' scale. It is assumed that
survey for Carey Rd section to be provided by the Town.
7. Complete design of the low pressure sewer,gravity sewer and flow meter. Coordinate with
City of Glens Falls for the flow meter and Broad St.sewer sections.
8. Assist with permitting and approvals of each section. Anticipated approvals will be
required by: City,Town,NYS DEC and Warren County.
9. Assuming that SEQRA has been completed for the project, no archeological impacts or
wetland impacts will result or stream crossing permits will be needed.
10. Complete final plans, specifications and bid documents for Town and City for approval.
Include NYS Labor Rate schedule in bid documents.
11. Assuming the disturbance limits for Carey Rd sewer will exceed one acre, a SWPPP Plan
will be prepared and a Notice of Intent issued for the NYS DEC prior to construction.
12. Complete bid services,attend pre-bid opening and provide bid tabulation. Review bids and
recommend award for Carey Rd and Flow Meter. Note,the Broad St.Sewer will not include
bid phase services.
13. It is assumed that no M/WBE participation is required for this project.
14. It is assumed that no temporary or permanent easements will be required for this work.
The proposed schedule for Phase I is as follows:
• Award of Contract(anticipated) 12/11/17
• Field Survey&Mapping 12/11-12/15
• Preliminary Design 12/18-1/12/18
• Coordinate/Review with City,Town 1/15-1/26
• Final Design 1/29-2/23
• Permitting&Approvals 2/26-3/23
• Bidding&Award 3/26-4/24

Phase II-Contract Award and Construction Phase Services will include the following tasks for
Carey Rd and the Flow Meter:
1. Assemble and distribute contract manuals for signature.
2. Attend pre-construction meeting.
3. Complete Construction Administration services.
4. Attend and organize periodic progress meetings.
5. Address construction issues,interpret contract documents.
6. Review and approval of submittals and payment applications.
7. Issue change orders as needed.
8. Prepare record drawings.
9. Provide full time construction observation services. Total hours will be 550 for the purposes
of estimating project budget. SWPPP inspections will be conducted as required.
