HVV is authorized by the New York State Department of Labor under license # 05-0220 to perform asbestos inspections, sample collection and surveys. Samples of each type of suspected asbestos containing material (SACM) were collected. All • samples were collected by a NYSDOL licensed Class D Inspector, in accordance with 12 NYCRR Part 56, (referred to as Industrial Code Rule 56) and 29 CFR Part 1910. The sampling was conducted on December 13, 2017. A copy of the NYSDOL Asbestos license is attached to this letter. Methods • Prior to initiating sampling, an on-site walk through inspection of the structures was performed in accordance with Subpart 56-5(e) of 12 NYCRR Part 56. Based on the site inspection, sampling of materials was performed to evaluate for suspect asbestos containing materials (SACM). Bulk samples were collected following standard sampling procedures. The samples • were placed in double sealed sample bags and delivered by Federal Express to EMSL Analytical for analysis under formal chain of custody procedures. Friable type materials were analyzed by Polarized Light Microscopy (PLM) via the NY State ELAP 198.1 method. Non-organically bound (NOB) materials were analyzed by PLM via NYS method 198.6 NOB. If the NYS method 198.6 NOB was inconclusive, the NOB materials were then analyzed by transient electron microscopy(TEM)via NYS method 198.4 NOB. 902 Route 146 (518)371-7940 Clifton Park,New York 12065 Fax(518)371-5885 Pre-Demolition Asbestos Survey 10 Wood Point Lane Queensbury,New York December 20,2017 Page 2 of 3 Inspection Results The inspection revealed a two-story, four-bedroom, !four-bathroom residence, approximately 4,500 square feet in size. The structure was of wood framed construction, on a concrete and stone foundation. The exterior was sided with wood and stone. All exterior trim was wood or metal.The windows were aluminum or wooden framed, with no window, glazing. All roofmg consisted of rolled rubber membrane with metal flashing. No suspected asbestos containing materials were encountered or sampled on the exterior of the structure. The interior construction consisted of drywall walls and ceilings with mudded joints throughout the interior with textured skim coat on ceilings and most walls, hardwood flooring was present throughout the interior except for ceramic tiled floors in the kitchen and the two lower level bathrooms. A finished basement area contained wooden floors and pine paneled walls. The residence was reportedly constructed in two phases with the lower levels being original construction form 1978 and the upper level added later'in approximately 1990. The basement utility room area was unfinished and consisted of stone and concrete construction with a poured concrete floor. Fiberglass and corrugated paper insulation wrap was present on water lines in the basement utility room but was not considered suspect asbestos materials and was not sampled. Suspect ACM materials identified in the interior of the structure included drywall and mudded joint compound in the lower level of the residence which consists of the original construction from 1978, drywall and mudded joints in the upper level of the residence constructed in 1990, and textured skim coating on walls and ceilings that was found to be homogeneous throughout the interior. Bulk samples were collected of 1) drywall from the upper level constructed in 1990, 2)joint compound from the upper level constructed in 1990,3) drywall from the lower level constructed in 1978, 4)joint compound from the lower level constructed in 1978, 5) textured skim coat on walls and ceilings throughout the interior. Asbestos Containing Material Sampling Results A minimum of three samples of each type of suspected ACM was collected. The samples and results are summarized in Table 1 below. The Analytical results and chain of custody are attached to this letter.
