NOTE.
The wastewater treatment system is designed and approved
based on the installation of water conserving fixtures and a
design f ow of 110 gallons per bedroom. The system is not
designed to accommodate extreme water use fixtures, such
as Jacuzzi— type spa tubs or water treatment equipment. The
system is not.designed to accommodate garbage grinders.
The installation of garbage grinders, non-conserving water
fixtures or extreme water use fixtures is contrary to the approval
of this wastewater treatment system.

Dear Mr. O'Brien,
The construction of a replacement sewage disposal field for the residence at 65 Meadow Drive
TM #297.17-1-32) has been completed. The installation was completed by Lou DeRidder of
Morning Star Septic Service, who was also present for the final inspection.
As the design professional of record, I certify to the following statements:
1. Engineering design report with specifications and details were prepared November 27,
2018 by Kevin L. Hastings, PE for a 4 bedroom maximum (440 gpd) single family home;
2.Design documents were compliant with the Town sanitary health codes, and the NYS
Health Department Appendix 75-A, as applicable;
3.Field observation of constructed works on December 12, 2018, by Kevin L. Hastings, PE
and prior to earth backfill of trenches and structures;
4.Completion of the work appeared to be in substantial conformance with the design
engineering report, with the following exceptions:
temporary erosion control along toe of fill slope to be maintained until stabilized;
final turf stabilization& lawn maintenance to be completed in 2019 by owner.
The as-built schematic of the replacement sewage disposal field with location ties to the dwelling
is attached for your records.

conducted on July 8,2019.
As the design professional of record, I certify to the following statements:

2. Design documents were compliant with the Town sanitary health codes, and the NYS
Health Department Appendix 75-A,as applicable;
3. Field observation of constructed works on December 12, 2018, by Kevin L. Hastings, PE
and prior to earth backfill of trenches and structures;
4. Completion of the work appeared to be in substantial conformance with the design
engineering report.
5. The previously disturbed construction work areas have been permanently stabilized. The
lawn grass appeared to be densely rooted and ready to be mowed.
The as-built schematic of the replacement sewage disposal field was previously submitted on
December 17, 2018. The attached photographs taken on-site during today's final inspection visit
are for your file.
