Re: Jim Barrett-437 Dix Avenue—Replacement Septic System
Dear Dave:
On April 1, 2008, we performed a deep soil test pit and percolation test at the Barrett Property
located at 437 Dix Avenue in Queensbury. The tests were performed to the northeast of the existing
business in the location where construction of a replacement leach field is proposed and was
observed by the Town Engineer Dan Ryan. The current business has an existing failed septic system
which will be decommissioned upon completion and certification of the replacement system.
The results of the testing are as follows:

4 —22" Fine Sand (Fill Material)
22 —27" Loamy Fine Sand with Gravel
27 — 55" Loamy Fine Sand
Mottling observed at 33"
Groundwater observed at 48"
The 18 inch deep hole was presoaked and the test was run 3 times; at a depth of 12 inches, until it
stabilized with three consecutive runs of 2:31, 2:45 and 2:48 respectively.
The design flow for this 29 employee commercial business at the NYS DEC flow rate of 15 gpd per
employee is 435 gpd and requires 458 sf of absorptive area. The replacement septic system should
be a shallow absorption bed system no greater than 6" below the existing grade. Prior to installation
of the absorption bed, the contractor must completely remove the gravel layer and backfill with an
loamy silty sand with a percolation rate of 1 to 5 minutes per inch.
The replacement shallow absorption bed system should consist of a new 1,000 gallon septic tank;
installed with 18 inches of soil over the cover, and a 4 foot diameter concrete pump station located
on the north side of the business. A 20 foot wide by 25 foot long absorption bed with a distribution
box & 4 —20 foot long laterals located to the northeast of the building.

Re: Barrett—437 Dix—Replacement Septic System
Dear Dave:
This letter is to inform you that we inspected the completed septic system for the Barrett property at
437 Dix Avenue in the Town of Queensbury on May 19, 2008.
The septic system as installed was for a commercial office building and consisted of a new 1,000
gallon septic tank, a new pump station, a new distribution box and a new 20 foot wide by 25 foot
long shallow absorption bed. The absorption bed was constructed with stone and 4 — 20 feet long
perforated pipe laterals. The system conforms to the requirements of the approved design letter.
