This office prepared a wastewater replacement design outline letter, dated July 7, 2009, (copy
attached) to Ellsworth Excavating for the above-referenced property. A representative from our
office visited the site during system installation. Based upon our one site inspection and our
discussions with the installer, it is my opinion that the system was installed in general
conformance with the design outline letter.
