The design plans, prepared by this office and reviewed and approved by the Town as associated with the wastewater system application,are as follows: Proposed Wastewater Improvements For The Berger Residence Applicant: Deb&Jim Miller • Sheet S-1 Cover Sheet FEB 2 6 2021 Sheet S-2 Existing Conditions Sheet W-3 Wastewater Plan TOWN OF QUEENSBURY • - Sheet W-4 Wastewater Details AS BUILT BUILDING&CODES Dated: October 3, 2019 During the course of construction I was on the job site to observe key components of the system installation. The Owner has continued the use of an existing ejector pump within the basement area. This installation involved a connection to the existing force main outside of the foundation with new force main upgradient to the new septic tank. The field location of the septic tank and the wastewater absorption system have been installed'as per the approved design plan., The D-box has been moved slightly so an 'As-Built' print has been provided for your file. Based on my inspection of the system start-up it is my opinion that the wastewater system is in compliance with the intent of the approved design plan and is ready for operation.
