EXECUTIVE SUMMARY PSI conducted a Demolition Asbestos Survey and limited lead based paint sampling at the P g Samoset Cabins Private garage and Hunters Hideaway Hotel Unit (27-30) located on Route 9 in ' Queensbury, NY, on May 11, 2004. The subject site consisted of the inspection of the Private garage and 1 hotel unit named Hunters Hideaway 27-30 located at the Samoset Cabins Resort. The Private garage was a wood frame structure with shingle roof and drywall walls. The ' Hunters Hideaway was a log structure split into 4 units. The logs have chinking mortar in between and the roof is asphalt shingle. All materials are similar between the units and it was reported they were all built in the same timeframe. The cabins were vacant during our survey ' activities. This survey was performed for The Great Escape Theme Park, LLC. This survey was conducted at the request of The Great Escape Theme Park, LLC. On the basis that the cabins will be demolished. ' The scope of the Demolition Asbestos Survey was to identify, t y, quantify and assess the condition of suspect asbestos containing materials (ACM) at the project site, as well as the collection and ' analysis of bulk samples. The assessment of these structures included the collection of 27 samples. Asbestos was not detected in any of the samples collected from the Private Garage or Hunters Hideaway units 27-30. Non-friable samples were further sent for analysis by Transmission Electron Microscopy, TEM and found not to contain asbestos as well. ' Please see the attached spreadsheets and analytical results for a listing of all materials sampled, their location and the analytical results. ' One paint chip sample was collected from the exterior brown paint of Hunters Hideaway cabin units 27-30. There was a detectable limit of lead found in these samples which was 0.097% ' Lead by weight. Please see the attached analytical results for sample location and analysis. ' 2 The Great Escape Theme Park,LLC Samoset Cabins Private Garage& ' Hunters Hideaway PSI Project Number 8214A086 INTRODUCTION ' 2. INTRODUCTION An Asbestos Demolition Survey and limited lead based paint sampling of the Samoset Cabins ' Private Garage & Hunters Hideaway, units 27-30 located on Route 9 in Queensbury, NY has been conducted by Professional Service Industries, Inc. (PSI) to identify Asbestos Containing Materials and potential lead based paint within the building. This survey was conducted on May ' 11 by Joshua Pierce, New York State certified asbestos inspector, representing PSI. The survey was conducted to prepare the building for demolition activities. ' The survey was generally conducted in four phases as follows: Phase 1 — Record Document Review- Drawings, floor plans, historical data or other documents provided o P p t PSI or made available on site were evaluated for the general construction history and layout of the facility. Other documents such as maintenance records, operation and maintenance plans, laboratory results, etc., provided to PSI or made available on site were also reviewed. This data was used to focus the walk through and scope of work to be followed over the course of our visual ' inspection and sampling. • • There was no previous data provided to PSI for this property. ' • Phase 2 — Visual Inspection- A visual inspection of the facility was conducted to identify, quantify and assess the condition of suspect ACM. The inspection team access each area ' and recorded suspect asbestos containing materials present. Each material was visually estimated for total quantity within the space. The general condition and friability was also recorded. PSI conducted intrusive inspection of areas behind finished walls and above ceilings in several areas. These intrusive evaluations were limited to representative areas of construction or areas where ACM was likely to exist. Extensive and exhaust investigation behind such closed systems was not requested or provided. There is a distinct possibility that conditions exist which could not be reasonably identified within the scope of this study. ' • Phase 3 — Sample Collection and Analysis- Samples were collected for each suspect Y P p homogeneous area. Samples were submitted to PSI's accredited laboratory for analysis by ' Polarized Light Microscopy (PLM) and Transmission Electron Microscopy, (TEM), where required. • Phase 4 - Project Report - This report outlines the assessment findings based on the interviews, testing results and field observations. The report also discusses other observations concerning the workplace as they impacted the sampling events. This report ' includes a discussion of sampling methodology, locations, analytical methods, results, and conclusions. 2.1 AUTHORIZATION Authorization to perform this asbestos survey was given by a signed copy of PSI proposal No. 821-4031R2 dated April 8, 2004, between The Great Escape, LLC and PSI. Access to the site was provided by Mr. John Collins of The Great Escape Theme Park, LLC. 2.2 SITE DESCRIPTION ' The subject site upon which this assessment was conducted is located on Route 9 in Queensbury, NY. The buildings included in this Demolition Asbestos Survey were the Samoset Cabins Private Garage & Hunters Hideaway units 27-30. The Samoset Cabins Private garage is a wood frame structure with asphalt shingle roof. The Samoset Cabins Hunters Hideaway units 27-30 is a log structure separated into 4 units. The roof is asphalt shingle. It is our understanding this property will be demolished. 2.3 PROJECT BACKGROUND PSI understands that the Samoset Cabins Private garage & Hunters Hideaway units 27-30 are scheduled to be demolished. As required an asbestos demolition survey was performed. ' 2.4 PURPOSE AND SCOPE The purpose of this asbestos survey was to identify accessible asbestos containing building materials and possible lead containing paints prior to demolition of the Samoset Cabins Private ' Garage & Hunters Hideaway units 27-30. The Demolition Asbestos Survey was completed in general accordance with the authorized ' scope of work as identified in the contract between PSI and The Great Escape Theme Park, LLC. ' 4 The Great Escape Theme Park,LLC Samoset Cabins Private Garage& ' Hunters Hideaway PSI Project Number 821-4A086 ASSESSMENT ACTIVITIES 3. ASSESSMENT ACTIVITIES ' The visual inspection and sampling activities were conducted on May 11, 2004, by Josh Pierce, asbestos inspector for PSI. Prior to the commencement of asbestos survey activities, The Great Escape Theme Park, LLC provided site contact information and assisted in providing access to the facility. 3.1 RECORD DOCUMENT REVIEW ' PSI was not provided floor plans, historical data, maintenance records, previous survey reports, laboratory reports or other documents for information regarding construction history and building materials. There were no materials provided by the owner. PSI was told all cabin units ' were built at the same time. ' 3.2 VISUAL INSPECTION PSI's inspector accessed each room or area of the subject site to identify suspect homogenous ' areas of ACM. Suspect ACM was categorized into homogeneous areas on the basis of color, texture, appearance, use and apparent construction era (where available). Each homogeneous area was given a unique material description. Quantities were visually estimated by the inspector. The inspection team selected representative areas to perform an intrusive evaluation of void spaces within the building or structure. Such inspections were made by creating an opening of sufficient size to determine the presence, condition and quantity of suspect ACM within. Void spaces which were evaluated included locations of suspected pipe or HVAC chases, wall cavities where fireproofing or other ACM was suspected, above finished ceiling systems where ' ACM was likely to exist. Although PSI made an attempt to identify all areas of ACM, an exhaustive investigation of void spaces was not included in the scope of services for this project. There may exist conditions which were unable to be identified within the scope of this ' study. In addition to identification of each material and quantities, the inspector also determined friability. A friable material is defined as any material able to be crushed, crumbled, pulverized or reduced to a powder by hand press when dry. The inspector used a hand pressure test to determine friability. Each material was further assessed for overall condition. Conditions were rated as good, fair or poor. Materials in good condition included those materials which were in ' the same condition as when installed showing only minor age deterioration. Materials in fair condition included those materials which had apparent age deterioration and minor damage; however the matrix of the material remained substantially intact. Materials in poor condition included all materials with damage or significant damage and evidence that the material's matrix has failed or has begun to fail. 3.3 SAMPLING AND ANALYSIS PSI's asbestos inspector, under the supervision of a Senior Technical Professional developed a sampling scheme for suspect ACM at the facility. 5 The Great Escape Theme Park,LLC Samoset Cabins Private Garage& ' Hunters Hideaway PSI Project Number 821-4A086 1 ASSESSMENT ACTIVITIES 1 1 PSI did not sample any system which presented a hazard to the inspection team such as energized electrical systems or within confined spaces, Each sample location was sprayed with amended water and was kept wet during the entire ' sampling process. Samples were collected by coring through the material from the surface down to the base substrate. All layers of the material were extracted in placed into a sample container for transport to the laboratory. Sample containers were sealed and labeled with a 1 unique sample id. Following sample extraction, the sample location was sealed using a clear liquid encapsulant or covered with tape. Restoration of finishes and materials to their pre- sampling condition was not provided. 1 Due to the destructive nature of roof sampling, PSI does not warrant a water tight condition following sample extraction, nor can PSI guarantee the continuance of any roof system 1 warrantees by other entities. Samples were submitted to PSI's laboratories in Schenectady, NY and Pittsburgh, 1 Pennsylvania. PSI's laboratories are accredited by the National Laboratory Accreditation Program (NVLAP) and New York State DOH ELAP. Samples were analyzed on a first positive stop basis to reduce the overall analytical cost. Samples were dried; homogenized and representative portions were examined with a stereobinocular microscope. If no asbestos is found in a sample, "NAW (No Asbestos Detected) is reported. If asbestos is found in a sample, the percentage and type of asbestos is ' reported. Point Counting verification of low concentration samples was performed. 1 1 1 i 1 1 1 1 6 The Great Escape Theme Park,LLC Samoset Cabins Private Garage& Hunters Hideaway PSI Project Number 821-4A086 CONCLUSIONS 4. CONCLUSIONS PSI has performed a Demolition Asbestos Survey and limited lead paint sampling of the subject site in general accordance with PSI Proposal No. 821-4031 R2, dated April 8, 2004. Based on ' the results of this assessment, the following conclusions have been developed. 4.1 ASBESTOS CONTAINING MATERIALS ' Asbestos was not identified in any of the samples collected from the Samoset Cabins Private garage or Hunters Hideaway units 27-30, located on Route 9oute 9, Queensbury, ' NY. During the inspection of the Samoset Cabins Private Garage and Hunters Hideaway units 27- 30, PSI collected 27 samples representing the identified suspect asbestos containing materials. ' A complete detail of all suspect materials, locations, quantities and conditions may be found in the spreadsheets found in the appendices of this report. ' Data Interpretation A material is considered an asbestos containing material if at least one sample from the homogenous area is confirmed to contain greater than one percent asbestos (>1.0%) under laboratory analysis. In addition, OSHA's construction standard considers all thermal systems insulation and surfacing materials in a facility constructed prior to 1981 to be presumed ' asbestos containing (PACM) and all flooring to be assumed asbestos containing unless it is demonstrated through laboratory analysis to contain 1.0% asbestos or less. The National Emissions Standard for Hazardous Air Pollutants further classifies ACM as regulated (RACM), ' Category I non-friable ACM or Category II non-friable ACM. LEAD BASED PAINT LIMITED SAMPLING ' PSI collected one paint chips for analysis of lead content from the brown painted logs exterior p 9 from Hunters Hideaway units 27-30. In the event other suspect asbestos containing materials not previously sampled are discovered during demolition, these materials should be sampled. PSI recommends following the OSHA guidelines for lead containing surfaces for detectible lead in painted surfaces. 7 The Great Escape Theme Park, LLC Samoset Cabins Private Garage& ' Hunters Hideaway PSI Project Number 8214A086 WARRANTY 5. WARRANTY Asbestos Survey The information contained in this report is based upon the data furnished by the Client and observations and test results provided by PSI. These observations and results are time dependent, are subject to changing site conditions, and revisions to Federal, State and local ' regulations. PSI warrants that these findings have been promulgated after being prepared in general ' accordance with generally accepted practices in the asbestos and/or lead-based paint testing and abatement industries. PSI also recognizes that raw laboratory test data are not usually sufficient to make all abatement and management decisions. As directed by the client, PSI did not provide any service to investigate or detect the presence of moisture, mold or other biological contaminates in or around any structure, or any service ' that was designed or intended to prevent or lower the risk of the occurrence of the amplification of the same. Client acknowledges that mold is ubiquitous to the environment with mold amplification occurring when building materials are impacted by moisture. Client further acknowledges that site conditions are outside of PSI's control, and that mold amplification will ' likely occur, or continue to occur, in the presence of moisture. As such, PSI cannot and shall not be held responsible for the occurrence or recurrence of mold amplification. ' No other warranties are implied or expressed. Use By Third Parties ' This report was prepared pursuant to the contract PSI has with Great Escape Theme Park, LLC. That contractual relationship included an exchange of information about the subject site that was unique and between PSI and its client and serves as the basis upon which this report ' was prepared. Because of the importance of the communication between PSI and its client, reliance or any use of this report by anyone other than Great Escape Theme Park, LLC, for whom it was prepared, is prohibited and therefore not foreseeable to PSI. Reliance or use by any such third party without explicit authorization in the report does not make said third party a third party beneficiary to PSI's contract with Great Escape Theme Park, ' LLC. Any such unauthorized reliance on or use of this report, including any of its information or conclusions, will be at third party's risk. For the same reasons, no warranties or representations, expressed or implied in this report, are made to any such third party. ' Unidentifiable Conditions This report is necessarily limited to the conditions observed and to the information available at the time of the work. Due to the nature of the work, there is a possibility that there may exist conditions which could not be identified within the scope of work or which were not apparent at the time of our site work. This report is also limited to information available from the client at ' the time it was conducted.. The report may not represent all conditions at the subject site as it only reflects the information gathered from specific locations.
