Attached are 2 copies of revised drawings for the above referenced project. I also provide the
following responses to your fax of 22 July 2004.
• The design loads have been added to dwg. A-1.
• Damp proofing is indicated on dwg. A-4.
• Foundation grade pitch is indicated on dwg. A-4.
• Smoke detector and carbon monoxide detector locations have been revised.
• The stair section in dwg. A-1 indicates the risers are 3/" wood risers.
• 1 have included a letter of acceptance from my observation of the septic system installation.
I have spoke with Jonathan Monroe who indicated that the drywell covers can be opened for
your inspection, the effluent pump and piping to the distribution box as well,as the
connection of the system to the new addition remains to be completed. I can provide digital
pictures of the drywell, septic tank and effluent pump pit installation if,they would be helpful.
I believe this addresses all the items contained in your fax. If there are any additional items please
let me know.

At the reference project, I observed the installation of the septic system including the septic tank,
distribution box and drywells prior to backfilling on 28 &29 June, 2004 and the system appears to
be installed substantially in accordance with the approved subdivision requirements and is
acceptable to this office.

Dear Ethan:
The following items need.to be addressed on the plans for Jonathan Monroe, so
that this Department can issue the permit.
1. The design loads of 90 mile an hour wind, 70 pound ground snow load,
and 30 pound floor load for sleeping area, 40 pound floor load other floor,
needs to be shown on the plans.
2. There is no damp proofing shown on the foundation cross section.
3. The foundation grade should indicate a six inch drop within 10 feet of the
foundation.
4. Smoke detectors should be located in the bedrooms, outside the hallways
adjacent to the bedrooms. It appears that there needs to' be a smoke
detector added, or moved, in each hallway adjacent to the bedrooms,
separated by the addition.
5. There is no note on the stairs as to whether they are closed risers or not.
6. A carbon-monoxide detector needs to be installed on the lower sleeping
room. I could find no evidence of that on the plans.
7. We have not inspected the septic system installed as of this date.
Once the above items have been addressed, this Department can issue a
permit for this project. Thank you for your attention in this matter.
