HVV is authorized by the New York State Department of Labor under license# 05-0220 to perform asbestos inspections, sample collection and surveys. Samples of each type of suspected asbestos containing material (SACM) were collected. All samples were collected by a NYSDOL licensed Class D Inspector, in accordance with 12 NYCRR Part 56, (referred to as Industrial Code Rule 56) and 29 CFR Part 1910. The sampling was conducted on February 29, 2016. A copy of the NYSDOL Asbestos license is attached to this letter. Methods The initial survey, dated March 21, 2016, identified joint compound in a sample collected from the basement stairwell. Since the result was positive the remaining samples were not analyzed (positive stop). It was determined that the entire dwelling, except the basement, had been remodeled at a later date. Consequently,the second follow-up survey was performed to evaluate the joint compound throughout the first and second floors of the dwelling. Prior to initiating sampling, an on-site walk through inspection of the structures was performed • in accordance with Subpart 56-5(e)of 12 NYCRR Part 56. Building plans were not available for review. Bulk samples were collected following standard sampling procedures. The samples were placed in double sealed sample bags and delivered by Federal Express to EMSL Analytical for analysis under formal chain of custody procedures. Friable type materials were analyzed by Polarized Light Microscopy (PLM) via the NY State ELAP 198.1 method. Non-organically bound (NOB) materials were analyzed by PLM via NYS method 198.6 NOB. If the NYS method 198.6 .NOB was inconclusive, the NOB materials were then analyzed by transient electron microscopy(TEM)via NYS method 198.4 NOB. 902 Route 146 (518)371-7940 Clifton Park, New York 12065 Fax(518)371-5885 I} Pre-Demolition Asbestos Survey 24 shore Acres Tax ID:227.17-1-10 Queensbury,New York June 21,2017 Page 2 of 3 Inspection Results The inspection revealed a two story, multi-bedroom, three bathroom, residence, approximately 3000 square feet in size. The structure was of wood framed construction, on a concrete block foundation with partial basement. The roof was mostly slate, with a small section of asphalt shingles over a seasonal porch. The exterior was sided with brick and clapboard siding. All exterior trim was wood. Most of the windows were wood framed with glazing. Window glazing was sampled during the prior survey and was determined to be ACM. The roof was slate, with the exception of a season porch,which had asphalt shingles with a tar paper underlayment. The Asphalt shingles and tar paper were sampled during the prior report and were determined to not contain asbestos. The interior construction consisted of drywall walls and ceilings throughout the entire interior of the structure. The floors were ceramic tile, carpet (over wood) and wood. The drywall was sampled during the prior survey. Twelve drywall samples were analyzed and were determined to not contain asbestos. Six samples of joint compound (dry wall mud) were collected throughout the first and second floors of the structure. This letter is meant to fulfill the requirements of NYS Department of Labor Industrial Code Rule 56 5.1 (h) which states: "No demolition, renovation, remodeling or repair work shall be commenced by any owner or the owner's agent prior to the completion of the asbestos abatement in accordance with the notification requirements of this Part."
