NOT IN GOOD REPAIR. A FIELD INSPECTION OF THE SEPTIC TANK WAS
COMPLETED BY MORNING STAR SEPTIC SERVICE, WHICH RESULTED IN A
FAILING REPORT. A NEW REPLACEMENT DISPOSAL FIELD IS REQUIRED FOR
CODE COMPLIANCE.

DESIGN NOTES & SPECIFICATIONS:
1) THE LOCATION OF THE REPLACEMENT DISPOSAL SYSTEM HAS BEEN
ESTABLISHED BY THIS DESIGN, AND ANY CHANGE IS SUBJECT TO REVIEW AND
APPROVAL BY THE DESIGN PROFESSIONAL PRIOR TO WORK. ALL DISPOSAL
TRENCH LATERALS SHALL BE OF EQUAL LENGTH. THE BASIS OF DESIGN
CALCULATIONS ARE PRESENTED ON PAGE 3 OF THIS REPORT.
2) THE CONTRACTOR MAY REMOVE ANY REMNANT PIECE(S) OF THE EXISTING
DISPOSAL FIELD(S) AS FOUND IN THE COURSE OF THE WORK. ALL WASTE
MATERIAL SHALL BE REMOVED FROM THE SITE FOR LEGAL DISPOSAL OFF-SITE.
3) ALL SEPARATION DISTANCES TO NEWLY INSTALLED ADJOINING TRENCHES
AND SETBACKS TO PROPERTY LINES SHALL BE MAINTAINED PER NYSDOH
APPENDIX 75A, AND TOWN CODES.
4) THE CONTRACTOR SHALL COMMUNICATE WITH THE ENGINEER OF RECORD
THE INTENTIONS OF HIS LAYOUT, PRIOR TO INSTALLATION, AND DISCUSS ANY
SITE LIMITATIONS THAT HAVE POTENTIAL TO INHIBIT HIS WORK.
5) THE CONTRACTOR MAY NOT PLACE BACKFILL MATERIAL (SAND, LOAM,
TOPSOIL) ON ANY COMPONENT UNTIL A FINAL INSPECTION HAS BEEN
COMPLETED BY THE ENGINEER OF RECORD AND THE TOWN CODE
ENFORCEMENT OFFICIAL.
6) ALL EXISTING SEPTIC TANK(S) AND/OR DRYWELL(S) IDENTIFIED ON THE
PLOT PLAN TO BE "ABANDONED IN-PLACE" SHALL BE DISCONNECTED AND
DECOMISSIONED, INCLUDING REMOVAL OF SEPTAGE, AND BACKFILLED WITH
COMPACTED GRANULAR SOIL.
7) AN AS-BUILT DRAWING OF THE SYSTEM AND ENGINEER'S CERTIFICATION
LETTER WILL BE REQUIRED PRIOR TO THE ISSUANCE OF A CERTIFICATE OF
OCCUPANCY FOR THE SYSTEM.
8) INSTALLED TRENCHES THAT REQUIRE MORE THAN 18" SOIL BACKFILL
COVER SHALL BE FITTED WITH AN ATMOSPHERIC VENT RISER AT THE DISTAL
END OF THE TRENCH, OR TRENCH SHALL BE BACKFILLED WITH AGGREGATE#2
CRUSHED STONE TO WITHIN -18" BELOW FINISH GRADE. UP TO 3 TRENCH VENTS
MAY BE INTERCONNECTED IN MANIFOLD TO A COMMON 4" PVC RISER,
TERMINATING WITH A COVERED VENT, 30"ABOVE FINISH GRADE.
