1. I acknowledge that no construction shall be commenced prior to the issuance of a.valid permit and will be
completed within a 12 month period.Any changes to the approved plans prior to/during construction will require
the submittal of amended plans, additional reviews and re-approval.
2. If,for any reason,the building permit application is withdrawn, 30%of the fee is retained by the Town of
Queensbury.After 1 year from the initial application date, 100%of the fee is retained.
3. Ifthework is not completed by the 1 year expiration date the permit may be renewed, subject to fees
and department approval.

As I mentioned on the phone, Behavioral Health Services North recently acquired 25
Willowbrook. BHSN occupies 1/2 of.the second floor and the other half is vacant (fully finished office
space formerly occupied by SCI). BHSN needs all of the second floor space for their use and would
like to join the spaces together by opening up one wall and installing a door (and to install 3 other
interior doors in existing hallways of the.space that's currently vacant). I've attached a marked up
floor plan which illustrates what they-have in mind. The project appears to be pretty simple on its face
but we want to confirm what requirements your office has in order to be zoning compliant.

• Verify fire extinguisher location and inspection
• Location of exit/Emergency lighting appear adequate. Function test will be
required
• Lock/ latches shall comply with Chapter 10 of 2020 IFC.
• Verify paths of egress
• NFPA 72 letter of compliance required for any alterations to the fire alarm
system
• NFPA 13 letter of compliance required for any modifications to the
sprinkler system
• Contractor to exercise caution and provide prote 'on for all fire alarm and
sprinkler devices, during construction
