Dear Mr. Boyea
We have,this day, approved plans for the construction of a conventional trench absorption system for
the referenced facility. This system was designed for 905 gallons per day and consists of 2-1000 gallon Grease
Traps, a 3000 gallon septic tank and a pump station which doses the absorption system. Please note that this
approval is conditioned upon the following:
a) that the proposed works be constructed in complete conformity with the plans and specifications accepted
this day or accepted amendments thereto;
b) that the construction is certified-by a professional engineer currently registered in the state of New York.
The certification shall state that construction was completed according to the approved plans and must be
submitted to Department of Health;

c) that this approval will expire two years after the date of issuance;
One set of the accepted plans and specifications is being retained in our files, and a set is being returned
