Dear Barbara&Francis:
This letter is to advise you that your addition currently under construction for your home located at
242 Meadowbrook Road must comply with the National Flood Insurance Program,instituted by the
Town of Queensbury.
In order to meet the requirements of the National Flood Insurance Program you must take steps
concerning your addition to reduce the potential for flood damage, as you are located within the
Flood Zone for Halfway Brook. •
Attached to this letter is an application with information that must be provided to this office by a
Licensed Surveyor on your behalf, along with some additional requirements. Those additional
requirements require that if you maintain the crawl space, as is currently shown on your plans, you
must create a minimum of two openings that have a net area of not less than one square inch for
every square foot of floor area for your addition. This means you would have to provide 840 square
inches of venting in your crawl space by means of louvers or some type of openable vents that would
allow water to pass through your crawl space in the event of a flood. These louvers or vents must be
provided at opposite ends of the crawl space,must be located no higher than one foot above finished
grade and be equipped with screens or some type of other device that would permit water to enter
your crawl space, in th&event of a flood, and exit the crawl space through the opposite louver or
vent. No utilities such as hot water tanks, furnaces, electrical panel box are allowed to be placed in
this crawl space.
If you have any questions regarding any of these requirements, please don't hesitate to contact me,
otherwise I would ask that you not hesitate to contact a surveyor and provide this information as
soon as possible.
If you have any questions concerning the Flood Insurance Program and the information required on
the application,I will be happy to sit down and go over it with you.

SCOPE OF WORK
1. Salvage Pressure Treated Deck.
A. Remove the stairs and add support to the deck.
B. Disconnect from the house and move out of the way.
C. Remove ramp section as needed for the deck.
D. Reconnect deck to the house and ramp.
E. Install stairs and tie in railings as needed.
2. Site Work.
A. Excavate for partial height basement above the water table.
B. Install gravel subbase for the slab area.
C. Apply foundation coating to the exterior basement walls.
D. Install footing drain around the addition and tie into the sump pit.
E. Apply 2"rigid foam insulation to the outside of the basement walls, down
to the slab elevation and apply a parge coating above grade.
F. Backfill foundation with the existing materials.
G. Dig for a new sewer line to the existing grinder pump.
H. Grade,hay and seed areas around the addition.
3. Concrete Work.
A. 8"x16"reinforced concrete footings at 48"min. below grade.
B. 8"thick reinforced concrete basement/crawl space walls.
C. 4"thick concrete slab with welded wire mesh reinforcing.
4. Rough Framing Work.
A. 2"xl0" floor joists and girder with 3/4"Plywood subfloor.
B. 2"x6" exterior framed walls with studs spaced at 16" OC.
C. 1/2" CDX plywood wall sheathing with Tyvek Housewrap.
D. Roof trusses spaced at 24" on center.
E. 5/8"CDX plywood roof sheathing with clips.
F. 12"wide overhangs on the low eaves and the gable ends.
G. 2"x4"interior framed walls with studs spaced at 16" OC.
H. Remove the exterior wall to connecting the addition to the house.
I. Modify and frame additional walls in the existing house as needed.

6. Exterior Windows and Doors.
A. Provide one additional all vinyl, double hung windows with
grilles and screens, similar to the existing.
B. Remove and reinstall five window units from the existing house.
C. Provide one bay window for the existing house.
D. Provide a 2'8"x6'8"Benchmark steel, insulated door with 1/2 glass.
E. Install a Larson Storm Door at the new back door.
7. Vinyl Siding and Overhangs,similar to the existing.
8. Insulation Work.
A. R19 Kraft-faced fiberglass insulation at the exterior walls.
B. R38 Kraft-faced fiberglass insulation at the ceiling with
venting at the low eaves.
9. Sheetrock Work.
A. Sheetrock work in the addition(Phase 1).
B. Sheetrock and patching in the house after breaking through
from the addition(Phase 2).
10. Finish Carpentry.
A. D-select pine window sills and extensions.
B. Stainable 6-panel pine doors for the addition
and existing house.
C. Stainable 3-panel pine door units.
D. One pair of pocket doors with 6-panel pine door leafs.
E. Stainable ranch casing and baseboard.
F. Shoe molding as needed in the kitchen area for the new flooring.
11. Painting and Staining Allowance.
A. Prime and paint ceiling and walls in the addition.
B. Prime and paint new walls in the existing.
C. Paint existing walls and ceilings disturbed by the modifications.
D. Stain, fill nail holes and polyurethane the interior doors,
window trim, and baseboard.

A. Remove the existing flooring as required in the house.

E. Flooring at the Existing Bathroom will remain.
13. Plumbing Work.
A. Copper water supply piping.
B. PVC waste and vent lines.
C. PVC sewer connection at the outside pump.
D. Vanity, fixture and faucet allowance of$825.
14. Heating Work.
A. Remove the existing furnace and make some ductwork alteration.
B. Provide a new system for the original house and addition including:
1) 92%Efficient horizontal hot air furnace.
2) PVC intake and exhaust pipes and fittings.
3) New sheetmetal ductwork for warm air supply
and cold returns.
4.) Digital programmable thermostat.
15. Electrical Work.
A. Lighting and outlet circuits.
