Hieghts in the town of Queensbury.

emergency:;basis;to: minimize;:expo sure, of'=the residents, genefa1_publi6,'pets''and°area
wildlife to thepotentially serious-.health hazard that a failed"wastewater 'disposal'system
poses.
On Tuesday, November 19, 2002, staff_ from this observed the new wastewater disposal
as it had been installed. . Additionally, Jarrett-Martin Engineers, PLLC staff verified the
in-situ soil percolation rate. The.-visual observation. by staff from this office was
facilitated by I.B.S. Septic &Drain. .I.B.S.' personnel excavated observation holes for the
following: building soil line, septic tank inlet, septic tank outlet, 'septic tank effluent line,
distribution box, distribution piping, head and distal end of each lateral (3 laterals).
Concurrently with the observation performed by this office, Dave,Hatin from the Town
of Queensbury Department of Building and,Codes performed a construction inspection of
the system for the town.
It is the opinion-of this.office that the wastewater-disposal system installed.'toserved Lot.
#15 .was 'installed,. substantially.,.in.-,accordance• with;New •York'�State..Dept: `of Health
