processing,the applicator must continuously observe the characteristics of the sprayed foam and adjust processing temperatures and pressures to maintain proper
cell structure,adhesion,cohesion and general foam quality.It Is the sole responsibility of the applicator to process and apply Heatlok HFO High Lift within specification.
General Requirements:Equipment must be capable of delivering the proper ratio(1:1 by volume)of polymeric isocyanate(PMDI)and poyol blend at adequate
temperatures and spray pressures.Substrate must be at least 5 degrees above dew point,with best processing results when ambient humidity Is below 80%.
Substrate must also be free of moisture(dew or frost),grease,oil,solvents and other materials that would adversely affect adhesion of the polyurethane foam.
Applicators should limit the application of this product to no more than a thickness of 6.5"(165mm)per pass(after expansion)to avoid fire hazards(including
spontaneous combustion)resulting from excessive heat generation.If subsequent passes are needed,applicators should waft until the core temperature of the foam
has dropped below 100°F to allow any reaction heat to dissipate from the prior applications before attempting to reapply the product.

In compliance with application approvals granted by the Town of Queensbury, we offer the
following regarding the referenced project.
The Site Plans prepared by EDP and approved by the Planning Board on May 26, 2021, involved
the construction of a single-family residence located at 3 Polk Road. Our office has been on the
site to observe the stormwater device installation, and all proposed stormwater management
devices are in place and functioning properly as designed. As there were slight changes to the site
construction during the course of the project, we have provided an As-Built Plan which identifies
the stormwater and erosion control measures implemented at this site.
Based on our inspections of the site improvements as installed, it is our opinion that the work
completed is in compliance with the intent of the approved design plans, has been properly
stabilized and is satisfactory for a certificate of compliance. If additional information is required,
please contact our office at your convenience.

During the course of construction, I was on the job site to observe key components of the system
installation. I have attached a plan of the design layout which is labeled (and serves) as the`as-built'
plan. The septic system components consist of a 1500 gallon concrete septic tank, a 1000 gallon
concrete pump tank with a duplex pump assembly, a time-dosed control panel and a pressurized
-stone absorption bed. The tanks and bed were constructed in the locations as noted on the
approved plan—Utility Plan Sheet 3. During the construction phase the Owner,with concurrence
from EDP and the Town of Queensbury Building Department, made revisions to the basic
components of the system. I have updated the Detail Plan (Sheet 7)to reflect the final component
details of the `as-built'system.
We completed a system test to the pressurized distribution laterals on September 23rd with the
Town representative (CD) in attendance. Alarm systems were checked and determined functional.
Floats and controls were set for system operation. Based on my inspection of the system start-up it
is my opinion that the wastewater system is in compliance with the intent of the approved design
plan as well as the subsequent as-built plans, and is ready for operation. The Owner is now looking
to receive a CO for this residence. Please contact me should you require any further information.
