SEPTIC SYSTEM LIMITATIONS
1. The best designed and installed septic disposal system will eventually fail to function properly without
adequate maintenance. Inadequate septic tank maintenance can result in clogging of the tank and I
sewage back-up into the home and/or sewage overflow onto the ground surface. The failure to 279 18-1-7 I
periodically clean a septic tank commonly results in clogging of the soil that surrounds the absorption -�
trench by overflowing solids not removed by the septic tank. �„�.� •�
2. This set of design plans and details is intended to provide subsurface sewage disposal for a typical 1..�
single family dwelling. No additions involving living or sleeping areas may be added to the dwellings
without the approval of the Code Enforcement Officer and the system redesign by a licensed Professional
Engineer. The seal and signature on any drawing specifically excludes certification for future expansion or
field replacement.
3. No garbage grinders allowed unless installed as initial construction of the house, and the septic tank
size increased by 250 gallons.
4. No sump pump, water softener backwash, spas, hot tubs, whirlpools, or condensate waste shall be
drained into the system. ' O
5. The dwelling shall have water saving fixtures, including but not limited to: 2 gpm shower heads, '
low water use toilets.
6. Observe water conservation practices.

10. Septic tank additives shall not be used in the septic system. Additives are unnecessary for the

17. The stamp seal and signature of the design engineer is for the intended use only, and any changeof use invalidates the seal.

A. Tank to be installed level on a 6" minimum layer of compacted gravel or stone;
B. Precast concrete tank, dimensions and features as shown;
C. Inlet baffle or 4" PVC tee to extend 16" minimum below flow line, 6" above flow line;
D. Outlet baffle or 4" PVC tee to extend 18" minimum below flow line, 6" above flow line;
E. Free air space above tees to be 3" minimum to top of tank.
F. Where installed, 4" plastic effluent filter cartridge with wooden handle extension to grade.
G. Grade access covers to be 24" integrated plastic or cost -iron cover, securely attached to tank.
7. Distribution box specifications:
A. Box to be installed level on a 12" minimum layer of compacted gravel or stone;
B. Precast concrete box, dimensions and features as shown;
C. Outlet speed levelers to be installed in the distribution box and adjusted to equalize the flow
to each lateral.
8. A permeable geotextile or non -woven filter fabric to be installed over the absorption trench areas to
prevent the infiltration of soil into the aggregate.
SEPTIC SYSTEM GENERAL NOTES
1. There shall be no change on these plans in advance of, or during construction, without prior written
approval of the design engineer.
2. The workmanship, construction practice, material standards, and inspection requirements shall comply
with the latest edition(s) of the following:
A. New York State Department of Health
1. New York Title 10. Department of Health. Chapter II. Part 75. Standards for Individual Onsite
Wastewater Systems
2. Appendix 75-A, Wastewater Treatment Standards -Residential Onsite Systems, March 16, 2016
B. Local town, village, or city sanitation ordinance(s).
3. The following inspections of the septic system by a licensed Professional Engineer shall include:
A. Fill material, at borrow pit or source of origin (perc. test may be required);
B. Fill material, in -place and finish graded;

4. Any modifications to the septic system design shall be performed by a licensed Professional
Engineer.

5. The installation contractor shall prepare the "as -built" sketch which has accurate dimensional

distances to each of the septic system components, as measured from at least two different physical
landmarks (i.e. foundation corners, well cap, boundary markers, etc.). This record information shall be
furnished to the home owner, Town, or other municipal officer, as required, and prior to occupancy of
the dwelling.
6. The septic system designs are typical size requirements for the number of bedrooms listed in the
design charts, and are based on established design flows and preliminary soil percolation rates
determined in the field. The installation contractor shall immediately notify the design engineer of any
field conditions which deviate from the information as shown on the approved plans.
7. The septic system designs shown were sized in accordance with the New York State Department of
Health standards, and local regulatory codes, as applicable.

Dear Mr.Poppa
The construction of a replacement sewage disposal system for the building at the above referenced
location Tax 9279.18-1-16 has been completed. The installation was completed by IBS Septic and Drain
Service on May 24 & 25, 2023.1
As the engineer of record, I certify to the Town of Queensbury as follows:
1. I am in fact the designer of the system.
2. I am personally familiar with the actual site conditions and in consideration of those conditions
the system was designed in compliance with the Town of Queensbury standards requiring two
feet of separation between the bottom of the tile trench and the annual high-water table and Part
75A of the New York State Department of Health Sanitary Code.
3. Wales Engineering staff, under my direct supervision, inspected the system at the time of
construction and I have personal knowledge that it was constructed in compliance with the
design. Further, there were no field conditions encountered during construction which deviated
from the conditions observed at the time the system was designed.
4. Wales Engineering prepared an asbuilt plan (see attached) depicting the septic system as it was
constructed, including measurements from: well to tank, well to leach field and d-box, adjoining
property wells within 200' of disposal field (if applicable), to tank and leach field, building to
tank, building to leach field, tank to d-box and leach field dimensions. Inspection performed on
May 25, 2023.
