Application for this project was duly made by yourself on plans titled, "Lot 134 & 135 Adirondack Street' and was most recently,received by this office May 10, 2023. The project includes the installation of a new onsite wastewater treatment system for Lots 134 and 135. The systems will consist of one 1,000-gallon septic tank and 150 LF of absorption trench to serve one 3-bedroom house each, with the existing 1,000-gallon septic tank at Lot 135 to remain. Approval of these plans is conditioned on the following: r 1. That the proposed works be constructed in conformity with the approved plans or amendments thereto; 2. That all components of the proposed works be installed, operated, and maintained as per the manufacturer's specifications; 3. That approval be granted by other agencies or municipalities have jurisdiction; 4. Certification shall also be provided to the Town Office Building and Code Enforcement. The purpose of this visit was to observe the installation of the replacement septic system designed by EDP (March 2023) and approved by NYSDOH (May 12, 2023). Based on review of the plans and observation of the installation, it is my opinion that the system was installed substantially in compliance with the intent of the design plans.
