The installation was completed by Dufour Excavating on 10.4.23. As the engineer of record,I certify to the Town of Queensbury as follows: 1. I am in fact the designer of the system. 2. I am personally familiar with the actual site conditions and in consideration of those conditions the system was designed in compliance with the Town of Queensbury standards requiring four feet of separation between the bottom of the absorption trench and the annual high-water table and Part 75A of the New York State Department of Health Sanitary Code. 3. Wales Engineering staff, under my direct supervision, inspected the system at the time of construction and I have personal knowledge that it was constructed in compliance with the design. Noted field variation that were encountered during construction which deviated from the conditions observed at the time the system was designed. *Proposed Septic Tank was run parallel with the side property line and the disposal field was changed from 3 laterals to 2 laterals @ 55'. The layout change was made due to lot size restrictions. This did not change design parameters.Lot is served by municipal water...House was not set a time of inspection. 4. Wales Engineering prepared an asbuilt plan(see attached)depicting.the septic system as it was constructed, including the appropriate measurements. Septic tank to property line, septic tank to street, disposal field to side setback.Inspection was performed on October 4,2023.Verification of proper setbacks to building,by others. WHEN GARBAGE GRINDERS ARE INSTALLED AT THE TIME OF HOUSE CONSTRUCTION, A GAS DEFLECTION BAFFLE AND A DUAL COMPARTMENT SEPTIC TANK, OR TWO TANKS IN SERIES, SHALL BE PROVIDED To MINIMIZE THE SOLIDS CARRYOVER TO THE ABSORPTION AREA (DISPOSAL FIELD). 2. 3- MINIMUM ELEVATION DROP THROUGH STRUCTURE FROM INLET TO OUTLET. 3. TANK SIZE REQUIREMENTS FOR MORE THAN SIX BEDROOMS (EQUIVALENT DAILY FLOW) SHALL BE CALCULATED BY ADDING 250 GALLONS AND SEVEN SQUARE FEET OF SURFACE AREA FOR EACH ADDITIONAL BEDROOM. A GARBAGE GRINDER SHALL BE CONSIDERED EQUIVALENT TO AN ADDITIONAL BEDROOM FOR DETERMINING TANK SIZE. A HOT TUB SPA SHOULD BE CONSIDERED EQUIVALENT TO AN ADDITIONAL BEDROOM FOR DETERMINING TANK SIZE. 4. EFFLUENT FILTER, "POLYLOK" BRAND OR SIMILAR PRODUCT TO BE ADDED TO THE SEPTIC TANK OUTLET. EFFLUENT FILTERS WHEN PROPERLY MAINTAINED CAN EXTEND THE USEFUL LIFE OF THE DISPOSAL FIELD. ALL SYSTEMS ARE SUSCEPTIBLE TO CLOGGING BY SOLIDS CARRY-OVER, LEADING TO PREMATURE FAILURE. IF INSTALLED, A GRADE ACCESS COVER SHALL BE PROVIDED FOR PERIODIC INSPECTION & CLEANING OF THE EFFLUENT FILTER DEVICE. * AVAILABLE AS SINGLE COMPARTMENT TANK ** AVAILABLE AS DUAL COMPARTMENT TANK 1 =30 EF1 COFY 100' MINIMUM (200' MINIMUM TO ANY UPSLOPE DISPOSAL SYSTE)J) TRENCH LENGTH - SEE BASIS OF DESIGN & LAYOUT DETAIL 10' MIN. 50' MINIMUM NOTE: THIS DETAIL ILLUSTRATES THE MINIMUM REQUIRED 9 20' MINIMUM SEPARATION DISTANCES FOR SANITARY FACILITIES ON A BUILDING LOT.
