TOWN OF QUEENSBURY AND EVOLUTION RECYCLING,INC.
FOR COMPUTER AND ELECTRONICS RECYCLING SERVICES
AND ADOPTING FEE SCHEDULE

WHEREAS, by Resolution No.: 44,2016, the Queensbury Town Board authorized an
Intermunicipal Agreement between the Town and Warren County and regarding Solid Waste and
Recyclable Disposal Services, Solid Waste and Recycling Transportation Services and
Electronics Recycling Services, with Evolution Recycling, Inc.,to provide Electronics Recycling
Services to the Town, and
WHEREAS, Evolution Recycling, Inc., has provided a Computer and Electronics
Recycling Proposal to the Town and the Town Board wishes to authorize its acceptance and
adopt an E-waste Fee Schedule for the Town of Queensbury/City of Glens Falls Transfer
Stations,
NOW,THEREFORE,BE IT
RESOLVED, that the Queensbury Town Board hereby accepts the Computer and
Electronics Recycling Proposal with Evolution Recycling, Inc., substantially in the form
presented at this meeting, and
BE IT FURTHER,
RESOLVED, that the Town Board authorizes the Town Supervisor to execute such
Proposal, as well as any other needed Agreement and/or documentation related to such Proposal,
and
BE IT FURTHER,
RESOLVED,that the Town Board hereby adopts an E-waste Fee Schedule for Town of
Queensbury/City of Glens Falls Transfer Stations as follows:
Charges for CRTs and flat panel units:
1 to 20 pounds - $ 3
21 to 40 pounds - $ 6
41 to 65 pounds $10
65+pounds $15
-No charges for all other electronic waste
and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor,
Landfill Equipment Operator, Recycling Supervisor, Town Budget Officer and/or Town Counsel
take any further action necessary to effectuate the terms of this Resolution.

RECYCLING SERVICE PROPOSAL
Evolution Recycling Inc. would like to officially offer our premier computer and electronics recycling
services to the Town of Queensbury,Ny. A summary of these services is as follows:

Insurance Evolution Recycling Inc.will provide and maintain NO CHARGE
all necessary insurance
Dedicated Account Management Experienced Evolution Recycling Inc. NO CHARGE
representative available to facilitate any need
Evolution Recycling Inc.will properly handle the
recycling of consumer electronics including
Recycling of Consumer Electronics keyboards,computer peripherals,VCRs,cable Pay.055/LB
boxes,game systems,computer towers,laptops,test
lab equipment,printers,etc.
Material Handling Training and Evolution Recycling Inc.will help prepare and train NO CHARGE
Consultation our customers how to properly handle their material
Safe and reliable transportation of product to
Transportation Evolution Recycling Inc.processing center. Pro Tek NO CHARGE
may also deliver recyclable materials upon notice to
Evolution Recycling Inc.
Receiving Reporting and C-7 Certificate Evolution Recycling Inc. will provide detailed
receiving reports of all product received for NO CHARGE
recycling
Environmental Compliance and Complete downstream vendor due diligence and NO CHARGE
Responsibility complete chain of custody tracking

RECYCLING SERVICE PROPOSAL (continued)
I. Scope of Service
- Full service CRT, computer and electronics recycling program including all of the features listed
above to the Town of Queensbury,NY.
II. Fees
> All fees are stated above in the Evolution Recycling Inc. service proposal. Amendments may
apply as agreed upon by both Evolution Recycling Inc. and the Town of Queensbury,NY.
III. Conditions
> Palletizing-
* Pallets- All CRT monitors, CRT televisions, flat panel televisions and printers must be
palletized on their own separate pallets. Therefore,monitors with monitors,
CRT televisions with CRT televisions, flat panel televisions with flat panel
televisions and printers with printers. Finally, a sufficient amount of stretch
wrap must be applied to each pallet to ensure the product is safe to move and
remain intact during transportation.
* Gaylords- All other acceptable electronics equipment may be placed into gaylord boxes
loosely by item type (keyboards with keyboards etc.). Gaylords must then be
double stacked(if possible) in preparation for transportation prior to Evolution's
arrival.
> Storage- The collection and storage of all materials, supplies and electronics equipment
must be done either inside or in such a way as to protect them from exposure to
outside elements.
> Volume- Evolution Recycling Inc. has no minimum or maximum volume requirment's.
ERI will pick up as requested for the full Fiscal year as needed.
> Duration- This proposal is valid from the date issued thru December 31st, 2016 with an
option to continue for an addition(3) one-year terms upon mutual agreement by
both parties.
> Reporting- Monthly. Billing will be completed within one week of receipt of materials.
