RESOLUTION AUTHORIZING ENGAGEMENT OF
THE CHAZEN COMPANIES FOR ENGINEERING REPORT AND GRANT
APPLICATION SEEKING FUNDING FROM NEW YORK STATE
ENVIRONMENTAL FACILITIES CORPORATION IN SUPPORT OF
MEADOWBROOK PUMP STATION FORCE MAIN LINING PROJECT

WHEREAS, the Queensbury Town Board wishes to engage The Chazen Companies for
preparation of an engineering report and assistance with completing a grant application seeking funding
from the New York State Environmental Facilities Corporation, which recently announced Round 2 of
the New York State Water Infrastructure Improvement Act, in support of the Town's Meadowbrook
Pump Station Force Main Lining Project, as delineated in Chazen Engineering's proposal dated
February 29t`, 2016 and presented at this meeting,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby authorizes and directs engagement of
The Chazen Companies for preparation of an engineering report and assistance with completing a grant
application in support of the Town's Meadowbrook Pump Station Force Main Lining Project, as
delineated in Chazen Engineering's proposal dated February 29t`, 2016 presented at this meeting, for
an amount not to exceed $7,500, and
BE IT FURTHER,
RESOLVED, that such services shall be paid for from Account No.: 032-8110-4720 in the
amount of$5,100, and Account No.: 036-8110-4720 in the amount of$2,400, and
BE IT FURTHER,
RESOLVED, that the Town Board authorizes and directs the Town Budget Officer to amend
the Town Budget to transfer $5,100 from 032 Fund Balance to 032-8110-4720, and $2,400 from 036
Fund Balance to 036-8110-4720, and take all necessary actions to effectuate payment, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor to
execute any necessary documentation and the Town Supervisor, Wastewater Director and/or Town
Budget Officer to take all action necessary to effectuate the terms of this Resolution.

Dear Mr.Harrington:
The Chazen Companies(Chazen)is pleased to provide this professional services proposal for preparing an
engineering report and grant application in support of a sanitary system improvement project for the Town of
Queensbury. The New York State Environmental Facilities Corporation(NYSEFC)recently announced Round 2 of
the NYS Water Infrastructure Improvement Act and the Town's Meadowbrook Pump Station Force Main Lining
project would be eligible for a grant award of 25%of the project costs.Applications are due on April 15,2016.
The following represents the tasks requested,and that we believe are necessary to accomplish your desired
objectives. Please refer to the Fee and Time Schedule Summary table near the end of this proposal for the costs
and time schedules associated with each task.
SCOPE OF SERVICES
Task 01—Engineering Report and Grant Application
Scope-Chazen,with assistance from the Town,will prepare and submit the required application documents for
potential grant funding for the force main lining project.
The NYS Water Grant Application requires the following documentation which will be prepared and submitted by
Chazen with assistance from the Town:
• Project Listing Form
• Engineering Report(with project budget and schedule)
• Smart Growth Assessment Form
• Completed CWSRF financing application
• SHPO Review(we will submit the initial Project Request for Review to SHPO)
• SEQR Review(action will be considered a Type II action)
• Bond Resolution(prepared and supplied by the Town's legal counsel)

Chazen will provide draft copies to the Town for review prior to submission.
Professional Services Fee Schedule
Chazen proposes to bill each task as indicated in the following Fee and Time Schedule Summary. Invoices will be
issued monthly for all services performed during that month,and are payable upon receipt.Lump Sum tasks will be
billed according to milestone completions for each deliverable,or commensurately with the percentage of the task
which has been completed.
