NOTE 1: All work is to be performed in accordance with manufacturees instructions, NYS
building, and local building codes. The contractor is responsible for all permits and inspections.
A Certificate of Occupancy or letter of compliance is required from the local Building & Codes
officer for final payment approval.
NOTE 2: UNLESS OTHERWISE NOTED,"INSTALL" MEANS TO PROVIDE AND INSTALL. ALL NEW

NOTE 3: All work is to be completed within a 2 month timeframe. Please keep this requirement in
mind when providing start and completion dates in your bid.
NOTE 4: For ROOFING; Contractor will be required to provide progress photographs showing
roof after stripped, after sheathing repairs and after installation of ice and water shield. This is
necessary for payment purposes.
Some or all of the rehabilitation work called for in these specifications is subject to HUD regulations
governing the reduction of lead-based paint hazards and must be accomplished pursuant to lead-
safe work practices.
LEAD-BASED PAINT HAZARDS
A lead-based paint risk assessment was done for this property and the resulting report indicates that all
painted surfaces were intact and also tested negative for lead-based paint. However, a minimal amount
of lead dust was detected on a bedroom windowsill. Therefore, a lead dust cleaning and clearance will
be required. Note that the clearance provider may take samples in areas other than those indicated in
the report. Refer to the attached report for details.
