following:

* A Sketch drawing entitled "'Plot Plan for lMiill and Jean Laurcesch", dated received by Queensbury
Sullding Department, November 17, 1993.
* Town of Queensbury Sufliding Permit dated November 18, 1199'3 for Installation of a septic system

* Certificate of Comipliance from Town of Queensbury certifying that the work shown on Permit
Application 93-7012 has been G0171pleteid,
