All work of this nature appears to be in compliance with the design intent and the construction documents or in accordance with discussions with the contractor during construction. There have been no major deviations from the construction documents and all work is acceptable to this office. • During installation of the sprinkler system it was discovered that an attic access panel to the truss space above the new seating area had been inadvertently omitted from the design drawings. This access panel is being installed in the gable end of the new addition and will be accessible from the low pitched roof above the existing lounge area. The attic space has been built as a "hot-roof" area and will have sprinkler system coverage. • The sewage disposal system installation has been completed and installation was reviewed by this office prior to backfill. All installations appear to be in compliance with the construction drawings and are acceptable to this office. As-built sewage disposal system drawings are attached to this transmittal for your file. Copies are also being provided to the Dept. of Health per their requirements. • A copy of the exhaust hood being provided for the new kitchen space along with the make- up air unit being provided is attached to this transmittal for your file. Please describe on plans and include justification that the existing system is capable of the increased load. 2. Please include the locations of all other septic systems in use on the plans. 3. Please change the 1500-gallon effluent pump chamber to 2000 gallons as discussed. 4. Please include contour lines in area of septic system. 5. Please include a separate heading for construction inspection notes in accordance with Appendix 75-A and the town of Queensbury. These notes should include which phases of construction are to be inspected, and by whom. —6. Please include a 50%reserve area`for the absorption field. 7. Please clarify if an effluent filter will be necessary at the outlet of the septic tank. 8. Queensbury, NY 12804 . Re: Application Review—Incomplete East Slope Ski Lodge,Facility Code 56-AD 19 Queensbury (T),Warren County Dear Mr. Barborne: This office is in-receipt of a Notice of Intent to Construct,Enlarge or Convert a Facility and Architectural Plans, completed by Rucinski Hall Architecture, received in this office on August 25, 2008 for the proposed addition of a 2'floor kitchen and bathroom at East Slope Ski Lodge, located at 59 West Mountain Rd, Queensbury (T). The portion of the Architectural Plans dealing with the water system and wastewater treatment system have been reviewed by Ms. Becky Dixon of this office, and comments regarding those plans forwarded under separate cover, also dated September 19, 2008. In reviewing the food service detail of the plans, this office finds the submission incomplete, and request the following issues be clarified by re-submission: • The Architectural Plans submitted have not been properly stamped by a Registered Architect. In speaking with you on September 19, 2008, you indicated a likelihood for modifications to the current submission, addressing issues relating to the installation of sinks in the 2nd floor kitchen. Please ensure the re-submission is properly stamped. Your plans indicate installation of gypsum board on the walls in the kitchen area, with paint as the intended finish. Please be aware that this substrate may not adequately perform to your needs in and around the area of the stoves and fryers. Hot grease from these units may permanently stain and/or damage this substrate, and therefore, we recommend installation of a more durable and easily-cleanable surface finish. • In discussion with you, you indicated that the suspended ceiling, originally detailed on the plans, is likely to be modified to painted gypsum. Please indicate this modification in your plan re- submission. • Please include detail regarding your proposed ventilation system for the newly constructed bathrooms, and indicate the installation of self-closing doors. • Please submit and updated menu for review. • In discussions with your design professional, it was indicated that you intent to alleviate the interior grease trap inside the kitchen. This office does not allow installation of interior grease P:\Facility Folders\East Slope Ski Lodge\Application Review-Incomplete.doc ' O traps in new construction food service establishments. Please indicate the removal of the interior grease trap in your re-submission. • You must include the equipment manufacturer technical specification sheets for all equipment to be installed in the kitchen. • Please re-submit your modified proposal for installation of sinks in the new kitchen. In discussions with you, you indicated that the currently proposed 3-compartment sink does not adequately meet your needs. Please be aware that you must, at minimum, install a hand wash sink in the kitchen area, and an additional deep basin sink, intended for convenient clean-up. General/Administrative • Please ensure a.Building Permit is obtained through your local Building Department, as was indicated in your submission. A copy of the Building Permit should be forwarded to this office with your re-submission. • Once construction of the facility has been completed, you must obtain a Certificate of Occupancy/Com fiance from theTocai Buiktiriepartment-or Code Enforcement Office. Please contact that office for further information and ensure that once approved, a copy of the Certificate of Compliance is forwarded to this office for filing. • Finally, not indicated on your submission, basic CPR equipment must be maintained at the 'facility. All work of this nature appears to be in compliance with the design intent and the construction documents or in accordance with discussions with the contractor during construction_ There have been no major deviations from the construction documents and all work is acceptable to this office. • During installation of the sprinkler system it was discovered that an attic access panel to the truss space above the new seating area had been inadvertently omitted from the design drawings. This access panel is being installed in the gable end of the new addition and will be accessible from the low pitched roof above the existing lounge area. The attic space has been built as a "hot-roof area and will have sprinkler system coverage.
