WHEREAS, the Town of Queensbury's Purchasing Agent sought proposals through the New
York State Contract Reporter for professional engineering services related to the Town's proposed
design and construction of an elevator at the Town Office Building, and
WHEREAS, six (6) proposals were received and reviewed by the Town's Purchasing Agent
and Facilities Manager and they have recommended engagement of the lowest, responsible bidder,
Lammon Architects, LLP, to provide the requested engineering services for an amount not to exceed
$12,400 as delineated in Lammon's June 26, 2018 proposal presented at this meeting, and
WHEREAS, the Town Board concurs with such recommendation,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby authorizes and directs engagement of
Lammon Architects, LLP to provide professional engineering services related to the Town's proposed
design and construction of an elevator at the Town Office Building for an amount not to exceed
$12,400 as delineated in Lammon's June 26, 2018 proposal presented at this meeting, and
BE IT FURTHER,
RESOLVED, that expenses for this project shall be paid for from Engineering Services
Account No.: 00 1-1440-4720, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor to
execute any necessary agreements and/or other documentation and the Town Supervisor, Facilities
Manager, Budget Officer and/or Purchasing Agent to take all action necessary to effectuate the terms
of this Resolution.

REQUEST FOR PROPOSALS
Elevator Design and Building Modifications
The Town of Queensbury is actively seeking proposals from qualified engineering firms for the design and
creation of specifications for the installation of a two level elevator system at the town's main office complex
1 located at 742 Bay Road, Queensbury, New York 12804
The selected engineering firm will be challenged to deal with existing infrastructure and blending both the
exterior elevator shaft as well as interior elevator appearances to match existing finishes.
It is reminded that if there are references in this document to specific equipment brands or specifications is
purely subjective and should be open to the engineering firm's verifications as validation.

=� A site inspection is scheduled for June 11, 2018 at 10:00 A.M. meeting in the Town Hall's atrium.
.� Interested firms must have proposals submitted to Joanne Watkins at the Town of Queensbury, 742 Bay Road,
Queensbury, New York 12804 by 4:00 P.M. on June 26, 2018 to be considered.
Engineering Firm Qualifications:
• Bachelor's degree or higher in mechanical engineering
• Licensed engineer in New York State
• Provide proven field experience

\� Daniel Pierce Library: Provided structural engineering services for a two-story addition to the historic
Daniel Pierce Library. Design elements included a full basement as well as two above grade floors.
The structure was constructed using reinforced concrete footings and foundation walls,structural steel
floor framing and columns,and pre-fabricated lite gage steel roof trusses. The new facility quadrupled
the size of the library and provided the community with much needed literary resources and
community education/meeting space.
James H.Maloy,Inc.: Provided pre-construction inspection services for multiple projects including
t projects in Downtown Schenectady and Ravenna. Tasks included extensive,visual inspections,
monitoring,and report generation of existing structures adjacent to the proposed work areas. The
purpose of these projects was to document the condition of the existing structures prior to
} commencement of the construction activities for comparison at the completion of construction
activities on the adjacent parcels.
Photovoltaic Systems: Over the past ten years,projects have included the planning and design of
} systems for the major PV providers in the area. Projects included structural inspection and analysis of

existing buildings,site layout and design for new ground mounted arrays,and construction over-site.

Mountain Lake Academy: As part of general facilities improvements and expansion,provided the
i analysis and design of a new educational building. The building was designed and constructed using
reinforced concrete foundations,structural steel framing,engineered lumber,and dimensional lumber
for the creation of additional classrooms.
Cambridge Central Schools: As part of a renovation to the High School Facility,provided analysis of
the existing gymnasium structural system for installation of roof mounted mechanical units,design and
' detailing of an addition to the gymnasium. Changing the use and space requirements for the existing
Locker rooms and classrooms,which required removal and realignment of columns and structural
members.
City of Albany,Load Rating Study: At the request of the City Engineer,performed visual inspections
for the purpose of collecting as-built data for mezzanine storage areas at a number of City facilities.
Based on the data collected,performed structural analyses to determine the maximum safe loading for
the storage areas which were summarized in a written report and presented to the City Engineer.

i This is to respond to a request made by Raymond Lammon for a letter of recommendation based on the
experience Hope House Incorporated has had with his Lammon Architects, LLP over the course of three
` separate OASAS funded capital projects.

The major project was the construction of a complete three story building designed to provide
residential treatment services for adolescents afflicted by substance dependency. The building was also
designed to house administrative office space on the ground floor. As the residential program is
Medicaid funded the building had to meet all applicable Albany City and State standards and codes. As
this building was constructed on six acres of land purchased by Hope House Inc.all exploratory zoning
and other land preparation work needed completion which involved work with City and State
representatives and ultimately the approvals of State OASAS and the City Common Council.
Other projects involved renovation of the basement of another separate older residential program for
emergency residential use. While the complexity and scope of this project was not as involved as the
residential program sited above,there was considerable pressure caused by funding limits,time
constraints and the requirements of governmental agencies.
1 Hope House Incorporated was granted funds for a second residential treatment program for adolescents
to be located on the same six acres occupied by the first residential program noted above. There was no
j question based on this agency's satisfaction with the design and functionality of the first building that
Lammon Architects, LLP would be selected to build,with only minor adjustments,an identical
construction for the second building. This building is targeted to start construction in the fall of 2013
and be occupied in 2014.

This is a letter of recommendation for Lammon Architects, LLP based on their
participation as part of the development team for a senior health project in the Capital
District. This project was complex in of itself, but it endured a longer feasibility period
due to the economic recession period in 2008 and 2009. Throughout the delay faced by
:} sponsors, Lammon Architects remained patient and committed to a project that found a
way to be financed with completion in 2012.
The project included renewal of a 1920's closed yet structurally sound school building.to
become a neighborhood located facility consisting of a state of the art senior day care
program, the program's administrative space, and suites for primary medical care and
rehabilitation therapy services which were a part of the facility that is licensed as a
primary care center under New York State Department of Health regulations. The work
by Lammon Architects, LLP was a significant part of the successful licensing application.
However structurally sound that the building was, conversion of a 1920's erected school
building presented a few challenges and surprises as a complete renovation of its
interior and heat, power, light, plumbing systems that along with new IT and
telecommunication systems created a modern environment designed for its new health
care mission. Lammon Architects was exceptional with taking the re-use of the building
into consideration with the architectural design and solutions. They were an integral part
of the project team, and very effective working with the contractor to realize the facility
on schedule and on budget. Throughout, they were conscientious with the client and
project team members and ever helpful with suggestions and effort to find solutions to a
problem that needed to be resolved. As a result, the Center building has received
exemplary compliments from health care professionals, members of the school's former
community, neighbors and from the program's sponsors.
Lammon Architects are a very reliable partner with a professional interest toward
community service projects and keen to see that the client is satisfied with an
architectural—facility project that can reflect a critical renewal and investment for the

The Town of Queensbury is actively seeking proposals from qualified engineering firms for the design and
.) creation of specifications for the installation of a two level elevator system at the town's main office complex
1 located at 742 Bay Road, Queensbury, New York 12804
The selected engineering firm will be challenged to deal with existing infrastructure and blending both the
exterior elevator shaft as well as interior elevator appearances to match existing finishes.
} It is reminded that if there are references in this document to specific equipment brands or specifications is
purely subjective and should be open to the engineering firm's verifications as validation.
A site inspection is scheduled for June 11, 2018 at 10:00 A.M. meeting in the Town Hall's atrium.
Interested firms must have proposals submitted to Joanne Watkins at the Town of Queensbury, 742 Bay Road,
