An inspection this date reveals the typical last minute activity of a business about to open. Final connections of electrical equipment, refrigerated cases and annunciator systems were being made. Per the October 8th report of Fire Marshal Bodenweiser, I observed the following: 1- Signs denoting Fire Extinguishers - Most are in place, but should be re-inspected when store is stocked completely. 2- Sprinkler head in Computer Room has been removed following permission of Fire Marshal. 3- Sprinkler head is in place in Flower Shop as directed. 4- All main sprinkler valves are in the "open" position and secured by chain & padlocks. 5- "No Smoking" signs are not yet installed. They are awainting shipment. I was accompanied by Bob Schools during my inspection. I suggested to him that one exit light, in the 2nd floor hallway serving the offices, should be mounted perpendicular to the wall for visibility purposes. He made note of this. It would be necessary to have a "resident inspector" on site. Our normal schedule of inspections does not apply here. We make every effort to visit the job-site as often as possible and attempt to view each portion of the structure during critical periods . Following is a summary of the past several inspections. Steel erection: Viewed fastening of vertical columns to foundation piers- Installation of girders , roof trusses, mezzanine framing and deck; also roof decking welding. All points of fastening appear to be proper. Roof; Material is on job-site; 21" foam insulation and elastomeric membrane. Membrane material bears BOCA Approval and has Underwriter ' s approval for external fire exposure. Plumbing: Several trips to inspect plumbing have shown the under-slab, cast iron piping to be installed in a workman-like manner. All joints that I have seen were fitted properly; laterals were pitched and vertical runs appeared to be plumb. Ditches for lines are being compacted as filled. 6" PVC Sch. 40 sleeves are being installed between concrete "pull boxes " for under slab refrigeration lines, etc. Floors: The first sections of forms for concrete floors are in place. Soils are compacted. Forms will accept 4" of concrete. 6"X6"X#10 wire mesh is on site for use in forms . Walls: As of 6/16/87, exterior concrete walls were complete. Expansion voids were not yet filled. Site work: Parking area is surfaced and curbing installed. Highway access lane is under construction. There are reports on file of the primary site preparation; as submitted by Morse Engineering. An inspection of the catch basins showed them to be clean and sub-surface water was flowing in the system. One concrete top, at the North end of the building, had been damaged. The weather was cloudy, cold, and about 30 to 35 degrees. The project sitework has been terminated until the springtime; however, the building foundation is being installed and is approximately 50% complete. The status of the project is as follows: 1. Post and rail fence installed along the property sides adjoining Doyles Nursery. 2. Topsoil remains stockpiled in loads within the parking lots flush medians. 3. Drainage pipe and structures installed on back side of building. 4. All lighting bases installed. 5. Lighting pole and fixture at northeast corner of building not installed, all other poles and fixtures installed. 6 . All areas to be seeded, east of the building site, have been topsoiled and rough graded. No seeding or planting accomplished. 7 . A patch of asphalt top course remains unfinished on entrance roadway from Quaker Road. 8. Asphalt top course has not been placed on entrance roadway from Bay Road, since it is being utilized as a haul road. 9. Detention pond completed, slopes rough graded, no seeding or planting accomplished. NOTE: Since the sitework is shut down for the winter season, this will be the last inspection report until further notice. Partnership and Clarence Carlsen of Einhorn Yaffee Prescott joined me at the site at approximately 10 :15 a.m. The septic tanks had not been delivered at this time. The inspector was off the site, back at 11 :30 a.m. , the septic tanks were not on the site at this time. At 1 :00 p.m. , the septic tanks arrived, the installation commenced, the large 5,000 gallon septic tank was installed in three sections. The grease trap was present and the distribution boxes present. All items were delivered by Fort Edward Express and dropped by Fort Edward Express. The contractor anticipated having all three structures set and backfilled on this date. Water will be pumped into the septic tank to prevent floatation. Asphalt concrete binder course (from Peckham Asphalt in Hudson Falls) was being placed by Green Island Construction, Inc. in the parking lot. Due to another commitment, the paving will not continue to be performed on Saturday, 11/1/86, as originally scheduled, but will continue on Monday, 11/3/86. 2. Topsoil has been placed and spread in all medians with granite curbs. 3. Topsoil has been placed and spreadon all areas adjacent to the entrance roadways, detention pond and parking area. 4. Granite curb has been installed for all the parking lot islands and the median of the Quaker Road entrance roadway. 2. The granite curb islands are being filled with topsoil. 3. Subbase material, adjacent to detention pond, was being graded and compacted. All other areas, roadways and parking area have been completed and are ready for paving. 4. Topsoil has been placed in piles in the at-grade separators of the parking lots. 5. Drainage structures have been adjusted: to grade in parking lot with frames installed but without grates. 6. Fill is being hauled to the site for the area of the building and area west to Grossman's. Area is being brought to final grade elevation. 7 . Site lighting has been almost completed, whereby all poles and fixtures have been installed on concrete bases and have been wired. One pole, base, etc. , at the rear of the building (west end) has not been installed, due to area being filled and raised to final grade. 8. Site grading and compaction was still in progress. 3. Underground wiring for the installed pole bases had been completed. 4. Entrance road to Bay Road and to Quaker Road had been constructed to subgrade and compacted. 5. Stripping of organic material behind proposed building was completed. Since there were not too many construction operations in progress, I met with Tom Spagnoletti of B.B.L. to discuss the schedule of work for the next few weeks. - N.Y.S. Item #4 will be delivered to the site and compacted. - The curb crews are scheduled to be on the site within the next couple of days to begin the curb installation. - The paving of the roadways and front parking area to be completed by the first week of November. The initial paving will only entail the binder course - the top course will be laid next year upon completion of the building. Project is progressing in accordance with the plans. End of inspection. NOTE: Tom Spagnoletti had heard that the proposed sanitary sewer to be constructed along Quaker Road will have a spur line which will be constructed along their Quaker Road entrance (in the area where planting is proposed) , loop around Doyle's and reconnect into Quaker Road. If this is the proposed route, he would prefer not to install some of the planting at this time. I introduced myself to Thomas Spagnoletti, Project Manager for Barry, Bette & Led Duke, Inc. , Contractors and . Engineers. I explained the firm of Morse Engineering would be performing periodic site inspections for the Town of Queensbury. Tom Spagnoletti discussed the project and what work has been performed to date, i. e. : 1. The site has been totally cleared and grubbed. 2. The site has been totally stripped, with the topsoil needed for the site stockpiled and the excess disposed of off the site. In some areas more topsoil was stripped due to there being more depth than originally anticipated. 3. The site has been graded to the approximate elevation of the subgrade. 4. Portions of the storm drainage system have been installed. 5. Uncovered in the location of the building was an area of unsuitable material, which is being removed, disposed of off the site, and acceptable material is being brought in for backfilling. 6. Empire Soils has been engaged to perform all the compac- tion tests and to determine the limits of unsuitable material removal. 7. Tom also explained the anticipated schedule of work to be performed until completion sometime in the middle of November. To date, the building has not been designed, although Hannaford Bros. , Inc. is presently negotiating with architectural firms. 1/A, October 8, 1986 M/E #86-111 Page Two Upon walking the site, there was a two-man survey party from Van Dusen-Steves checking control stakes, two workmen working on the storm sewer line, which is directed to Bay Road, and one front end loader with operator backfilling the same storm sewer trench. There was a backhoe with operator excavating unsuitable site material, bulldozer with operator spreading backfill material into excavated area, plus about three tandem trucks delivering backfill material and hauling unsuitable material from the site. As previously stated, the total site has been graded to approximately the subgrade elevation. The storm water system in the front parking lot, C.B. #5, C.B. #5A, C.B. #6, and C.B. #6A, with connecting pipes and outlet pipe to detention area have been installed and backfilled. The catch basins are open and are awaiting frames and grates. The storm water system which is directed to Bay Road, C.B. #1, C.B. #2 and C.B. #3, with connecting pipes and outlet pipe have been installed and were being backfilled. The roadways to Quaker Road and to Bay Road have been graded to subgrade. Subbase material for the roadways and parking areas has been stockpiled on the site. The storm water detention area has been graded and completed except for topsoil being placed. The two outlet pipes (8" and 18") for the detention area have been installed and backfilled. In the approximate middle of the building location is the area of unsuitable material which is being removed and backfilled with suitable material. This will be completed either today or tomorrow. It was observed that the excavation trench was filled with water to a height of approximately 3 to 4 feet below the ground surface. Upon completion of this excavation, the installation of the storm water system behind the proposed building will begin. It should be noted that Tom Spagnoletti stated that the building is anticipated to be larger than shown on Sheet No. L-2. They are considering expanding the building in the westerly, southerly, and easterly directions. End of Site Inspection.
