79 Glenwood Ave SPR Application Narrative 79 Glenwood Avenue, Queensbury, New York Site Plan Review Project Narrative Since 1967, The Addictions Care Center of Albany (ACCA) has treated and housed chemically dependent individuals and families. The Addictions Care Center of Albany empowers individuals, families and communities to advance beyond addiction and overcome barriers to healing through an array of quality, compassionate substance use disorder care, recovery, and prevention programs. ACCA respects each individual's unique journey through recovery and believes the disease of addiction is both preventable and treatable. ACCA has a diverse workforce, many of whom have lived experience, a strong administrative staff and a committed Board of Directors. ACCA has been awarded a contract by Warren County government for the provision of residential substance use disorder treatment and recovery services. ACCA plans to provide these services at 79 Glenwood Avenue, Queensbury, NY. The program will house 18 female residents, 18 years of age and older. Three levels of residential care as defined by the New York State Office of Alcoholism and Substance Abuse Services (OASAS) will be provided in accordance with PART 820 RESIDENTIAL SERVICE (Statutory Authority: Mental Hygiene Law Sections 19.07(e), 19.09(b), 19.40, 32.01, 32.07(a), Public Health Law Section2781. The three levels of care are: 1. Stabilization – At this level individuals will receive medically-directed Ancillary Withdrawal to manage mild to moderate symptoms of withdrawal. It is an alternative that provides a safe place to stabilize and engage in treatment. Medical staff monitor withdrawal symptoms. 2. Rehabilitation – will provide services for individuals who are stable enough to manage emotional states, urges and cravings, co-occurring psychiatric symptoms, and medical conditions within the safety of a residential setting. Services include medical monitoring of chronic conditions, including routine medication management and individual, group, and family counseling focused on rehabilitation. 3. Community Integration – Individuals are provided a safe living environment with a high degree of behavioral accountability. At this level individuals further develop recovery skills and prepare to transition into the community. Staff will coordinate treatment services but do not provide as high a level of clinical care. Most services include recovery oriented services; employment readiness and assistance with permanent housing. The residential program will be staffed 24/7. Staff include a fulltime Clinical Program Director (NYS Certified Addictions Professional), full-time Licensed Mental Health Counselor/Family Counselor, 1 Medical Director with scheduled onsite hours and on call; and a Psychiatrist with regular office hours. There will be a Registered Nurse on-site daily; and Licensed Practical Nurses and Case Managers 24/7. In addition, vocational skills development, art therapy, fitness activities, and family support will be regularly available. Nutritional meals will be provided on-site by a Food Service Manager. All of the above mentioned services are provided on-site. Residents will be transported to outside appointments which include primary care and specialty healthcare services, and to comply with legal requirements where they may exist (Family Court, Drug Court). ACCA is an experienced residential substance use disorder treatment provider. ACCA’s current continuum of care includes a 20 bed women and children’s community residence, 24-bed community residence for men, 24 men’s supportive living units and 24 women’s supportive living units. Also, ACCA was recently awarded a residential services contract in Rensselaer County for 22 males. In addition ACCA operates a transitional living complex for 12 men and a second transitional residence housing 17 women with up to 10 children of recovering mothers. The 20 bed women and children’s residence opened in April, 2018 and provides the same levels of residential care to be provided at 79 Glenwood Avenue. Other ACCA services include an outpatient clinic that maintains a full-day rehabilitation tract, intensive outpatient services as well as evening outpatient services. ACCA began providing specialized Home and Community Based Services, and Peer Support Services in 2017. ACCA delivers a comprehensive community education program throughout Albany County and the Capital District. Our Prevention Programs provide comprehensive substance abuse, bullying & gambling education in Capital District schools and community centers. Programs are designed for grades K - 4, middle school, high school and family-focused prevention programming, as well as afterschool programming during the school year and in parks and recreational sites during the summer months. The school based curriculums are delivered once a week, during a 45-minute session, for eight weeks. More than 5500 area youth benefit. ACCA is recognized by the New York State Office of Alcoholism and Substance Abuse Services (OASAS) as a certified professional training provider, and offers the 350 Hour CASAC Credential Training program and a variety of Continuing Education programs for addictions professionals, and addiction peer service providers. 2
