This office prepared the design for the wastewater system at the referenced residence.
On January_19, 2010, 1 visited the site and reviewed the details of the installation with
owner Adam Fitzpatrick. The system was completely installed and backfilled at this
time was not visible for detailed inspection.
Based upon my field review and discussions with the owner, it is my opinion that the
system is installed in general conformance with our design drawing dated 12/29/08.
