P.O.Box 4139 Queensbury,NY 12804 September 30,2008 Mr.John O'Brien,Building Inspector Town of Queensbury 742 Bay Road Queensbury,NY 12804 Re: Septic System Certification;GRJH,Inc. Exit 18 Service Station-107 Main Street Dear John; In accordance with the Town of Queensbury requirements, the septic system constructed on 107 Main Street for GRJH.Inc.is installed and certified as follows: 1. That I am in fact the designer of the system. 2. That I am personally familiar with the actual site conditions, and in consideration of those conditions, the system was designed in accordance with the Town of Queensbury Standards and in compliance with Part 75A of the New York State Department of Health Sanitary Code. 3. I have personally inspected the system at the time of construction, and I have personal knowledge that the system was constructed generally in conformance with the design. An existing 6' diameter drywell was encountered during construction, apparently ufiii7ed for"clean"water such as storm water, and as such is suitable for use for septic disposal. I measured the leach areas and computed the equivalent leach area for the drywell, and determined that by utilizing the existing drywell, the minimum design infiltration area is provided. 4. This certification applies to the attached 81/2" x 11" Drawing entitled"Septic System As- Built,GRJH,Inc.-Exit 18 Gasoline Service Station,Main Street",by Paragon Civil Engineering,P.C.dated September 30,2008. GRADE PLANE: A reference plane representing the average of finished ground level adjoining the building at exterior walls. Where the finished ground level slopes away from the exterior walls, the reference plane shall be established by the lowest points within the area between the building and the lot line or,where the lot line is more than 6 feet from the building, between the building and a point 6 feet from the building. 9. MFZZANINE: An intermediate level or levels between the floor and ceiling of any story with an aggregate floor area of not more than 1/3rd of the area of the room of space in which the level or levels are located. (Mezzanine area does not contribute to the building area or the height in stories but does contribute to the fire area, aggregate area). 10. SMOKE BARRIER: A continuous membrane, either vertical or horizontal, such as a wall, floor,or ceiling assembly, that is designed and constructed to restrict the movement of smoke. (Required to have a 1 hr.fire rating). 11. STORY ABOVE GRADE PLANE: Any story having its finished floor surface entirely above grade plane, except that a basement shall be considered as a story above grade plan where the finished surface of the floor above the basement is: 1. More than 6 feet above grade plane or 2. More than 6 feet above finished ground level for more than 50%of the total building perimeter or 3. More than 12 feet above the finished ground level at any point. Page 9 of 10 rage or cC [ FOR: GRJH, Inc.
