Dear Mr. Lawless,
As requested, GYMODPc completed an Asbestos Survey on the roof at the above referenced site. Our
NYS Certified Asbestos Building Inspector, John Phelps Cert # 09-08578 and Shana Toro Cert # 17-
37458, visited the site on February 06, 2018 and collected samples of roof building materials suspected
of being asbestos containing. The intent of this asbestos survey is to give an assessment of the suspect
materials that are going to become disturbed during the upcoming renovation. These samples were sent
to Paradigm Environmental Services for laboratory analysis. Based on laboratory analysis of the samples
collected, No(0) roofing materials tested positive for the presence of asbestos. The samples that where
tested are included in attachment A.
If other building materials are identified and shall be disturbed, the materials shall be presumed to be
ACMs until examined by an appropriately trained & certified individual and laboratory analysis proves
otherwise.

Dear Mr. Lawless
As requested, GYMODPc completed an Asbestos Survey at the above referenced site. Our NYS Certified
Asbestos Building Inspector Mitchell Blackwell Ceti#17-25242 visited the site on January 15, 2019 and
collected samples of building materials suspected of containing asbestos. These samples were sent to
Paradigm Environmental Services for laboratory analysis. Based on laboratory analysis of the samples
collected, none(0) of the materials tested positive for the presence of asbestos.
A list of all materials sampled can be found in Attachment A. If other building materials are identified and
shall be disturbed, the materials shall be presumed to be ACMs until examination by an appropriately
trained &certified individual and laboratory analysis prove otherwise.
