qualified professional consultant for expert review and recommendation arising from the carrying out of the provisions of
Art.IV,including,but not limited to,the review of plans,specifications and reports,attendance at inspections,dye tests,deep
hole test pits, septic related deep hole and percolation tests, system installations and any other aspect of any matter
contained in this Art.IV,and any costs incurred in such review shall be paid to the Town by the owner of the property before
any approval can be granted under this Art. IV shall become effective within 30 days of presentment of an invoice for same,
whichever is sooner.Such costs shall not exceed$1500 without proper written notice to the party to be changed the same.

Re: Basis of Design Report (BODR)
Replacement Septic System for
Jaikeran Single Family Residence at 394 Dix Avenue, Queensbury
Dear Pat:
The contractor has retained the engineer (Edward K LaPoint, PE) to provide a Basis of
Design Report for the above referenced project. Specifically;
1. The home is a two story, three bedroom wood framed building with a failed cesspool
in the front yard. The contractor is proposing a replacement septic system in
accordance with the New York State Department of Health's Appendix 75-A.
2. The system will consist of;
1. A new properly constructed ventilated house sewer, trap, and clean out.
2. A duel chamber pre-cast concrete 1,000 gallon septic tank with sanitary tees.
3. A pre-cast concrete distribution box with speed levelers.
4. Three 60 foot conventional absorption trenches (180 foot total).
3. A six foot deep test pit was dug to complete the soil investigation. The excavation
reveals;
1. Ten inches of top soil and miscellaneous fill.
2. Sixty-two inches of loose sand, gravel, and silt.
3. No groundwater or mottling was observed.
4. No bedrock was encountered.
4. Two separate percolation tests were run after the holes were pre-soaked for 24
hours..
1. Test Pit#1 - 1 m31 s, 3m3s, 4m13s, 3m28s, 5m9s.
2. Test Pit#2 - 5m23s, 6m38s, 5m30s, 5m48s.
5. For design purposes, the engineer used a stabilized percolation rate of 6 minutes
per inch, resulting in 180 feet of conventional absorption trench.

As requested, Chazen has witnessed onsite soil testing for the above referenced location on July 23, 2019. The
purpose of this testing was to investigate subsurface soil conditions for a proposed wastewater system design.
The wastewater system is proposed as a new system to replace a failed cesspool. The testing was performed by
Edward K. LaPoint, P.E.
One soil test was performed in the general area of the proposed septic system adsorption area.
