Town of Queensbury 745 Bay Road Queensbury,NY 12804 Attn: Mr. Dave Hatin RE: Angio Dynamics Gentlemen: This is to confirm that I have reviewed and approved two structural engineering changes made during the construction of the foundations for the new building at Angio Dynamics. On Drawing F-2, Detail #3 incorrectly shows #6 @ 12" rebar extending from the foundation into the masonry wall. The original design called for #6 bars at 24" on center. The foundation was constructed in accordance with the structural design using Ws @ 24" on center. On Drawing F-1, the Footing Schedule calls for piers on all interior columns. The contractor asked if, on the columns for the mezzanine framing, he could eliminate the piers, raise the footings and put the anchor bolts directly into the footing. After reviewing this I approved the change from a structural engineering standpoint. Please call me if you have any questions. Si cer 1 , Q Thomas W. We need drawings or specifications showing that fresh air ventilation will be provided to all spaces in the ventilation system. 2. The mezzanine needs two exits that will meet the maximum distance to travel of 150 feet. It appears that an exit will have to be made closer to the mezzanine, and that one stairwell exiting the mezzanine must be fully enclosed in one hour construction. 3. The equipment layout for the main production floor needs to be shown on the drawing or aisles shown that will lead to exits to provide a clearer path to the exits without interference of equipment or product. 4. We need dimensions on the stairs that show seven inch rise, eleven inch tread. 5. A note on the drawings that states that all glass within doors and glass adjoining doors will be safety glass or tempered glass. 6. The warehouse needs to be separated from the production area by a three hour fire separation. We need this shown on the plans as well as a detail of how that fire separation will be constructed. T. The mechanical room needs tQ have a two hour enclosure around it with an entry vestibule. We,need details showing wall construction in this area. The vestibule must be vented to the exterior. 8. All exit corridors must be three quarter hour rated. We need construction drawings showing how the corridors will be constructed in those areas. 9. The janitor's closet must be one hour rated. We need drawings showing one hour construction around the janitor's closet. 10. Fire stopping is required at 5,000 square feet above any concealed space or ceiling. We need a drawing showing the location of the fire stopping and a detail showing how the fire stop will be constructed from the ceiling through to the underside of the roof. 11. We need a note on the drawing stating that the sprinkler system will be installed according to NFPA 13 Standards and be submitted prior to installation of the system for Fire Marshal approval. 12. We need locations of pull stations and horn strobes shown throughout the addition, as well as the existing building,if they are not currently in the existing building. 13. A note on the drawing that states that all ventilation systems will shut down upon activation of the fire alarm system or activation of the sprinkler system. 14. We need the design loads shown for floor, snow and wind load, including the mezzanine area, equipment area, for the addition. "HOME OF NATURAL BEAUTY . . . A GOOD PLACE TO LIVE" SETTLED 1763 15. A note on the drawings that states all plumbing above slab will be cast iron or copper. All plumbing below may be PVC or cast iron. 16. We need to know the BTLJ ratings and duct locations of all mechanical equipment, and any mechanical equipment that penetrates a two or three hour fire separation must be provided with fire dampers at those locations. 17. We need an emergency light, exit light layout of the entire building. 18. All exits must be ramped to grade and have canopies covering the door swing. 19. The Check Mate, ComChek energy compliance forms must be submitted with the drawings to show energy compliance for this building. I believe this covers all items that were reviewed on July 25,2002. Once these items have been completed on a drawing, they will be reviewed by this office for compliance. If you have any questions, please don't hesitate to contact me. The proposed warehouse system utilizes ESFR heads while the production area is to be protected by extended coverage heads. The extended coverage heads are only suitable for use in Ordinary Hazard(or lower) occupancies. As reported, the production area will consist of assembly operations and does not include machinery that uses combustible hydraulic fluids. If combustible hydraulic fluids or substantial amounts of 1. flammable liquids are in use in this area or if product is stored to heights greater than 12 feet, the hazard classification changes to Extra.Hazard and extended coverage heads are no longer suitable or adequate. We rely on internal Chubb standards as well as the standards provided by the National Fire Protection Association(NFPA) when performing plan reviews to determine suitability for insurance purposes. The system designs indicated above are adequate for the indicated occupancy of this facility. The proposed systems meet my interpretation of the requirements outlined in NFPA 13.
