Replacement Septic System inspection

This letter is to inform you that we inspected the completed septic system for the Harvey
Residence on Chestnut Ridge Road in the Town of Queensbury on August 20, 2007.
The septic system as installed was for a four bedroom house and consisted of the new 1,250
gallon septic tank, a new distribution box and a new 20 foot wide by 35 foot long absorption bed.
The absorption bed was constructed with stone and 4 — 30 feet long perforated pipe laterals. The
system conforms to the requirements of the approved design letter.

Dear Dave:
On July 26, 2007, we performed a deep soil test pit at the Dave Harvey residence located at 92
Chestnut Ridge Road in Queensbury. The percolation test was performed by us on 7/6/2007. The
test was performed to the east of the existing residence in the location where construction of a
replacement leach field is proposed and was observed by the Town Engineer Dan Ryan. The
residence has an existing failed septic system which will be decommissioned upon completion and
certification of the replacement system.
The results of the testing are as follows:

0 —5" Topsoil Stabilization percolation rate - 1" in 8 min., 12 sec.
5 —32" Loamy Medium Fine Sand w/ some Gravel
32—80 " Fine Sand Compact
Mottling observed at 40"
No Groundwater observed
The hole was presoaked and the test was run 5 times until it stabilized with three consecutive runs
of 9:47, 9:58 and 10:12 respectively.
The design flow for this 4 bedroom residence at 110 gpd per bedroom requires 628 sf of absorptive
area. The replacement septic system should be a shallow absorption bed system no greater than 12"
below the existing grade. Prior to installation of the absorption bed, the contractor must completely
remove the topsoil layer.
The replacement shallow absorption bed system for the proposed residence should consist of a new
1,250 gallon septic tank located on the east side of the residence and a 20 foot wide by 35 foot long
absorption bed with a distribution box & 4 — 30 foot long laterals located to the east of the
residence.

Dear Dave:
We are currently working with Mr. Dave Harvey to design a replacement septic system for his
property at 92 Chestnut Ridge Road in the Town of Queensbury. (See map attached) The existing
septic system has failed and we are requesting your approval for out of season high groundwater
determination so that we can continue with the replacement septic system design. As you can see on
the map provided, we have field measured all boundary conditions and we feel that we can install a
replacement septic system without any variances.
We would like to schedule this work as soon as possible so that we can complete our design, submit
to the Town of Queensbury Building Department for a Septic System Permit and start construction
as soon as possible.
