RESOLUTION AUTHORIZING AGREEMENT WITH WELLCHOICE,
INC. FOR PROVISION OF ADMINISTRATIVE SERVICES FOR RETIREE
DRUG SUBSIDY PROGRAM ADMINISTERED BY CENTERS FOR
MEDICARE AND MEDICAID SERVICES

WHEREAS, the Town of Queensbury has requested that Wellchoice, Inc., on behalf of its
subsidiaries, Empire HealthChoice Assurance, Inc., provide services related to the Town’s
participation in the retiree drug subsidy program administered by the Centers for Medicare and
Medicaid Services, and
WHEREAS, a proposed Agreement between the Town of Queensbury and Wellchoice,
Inc., to Provide Administrative Services for the Retiree Drug Subsidy is presented at this
meeting,
NOW, THEREFORE, BE IT
RESOLVED, that the Queensbury Town Board hereby approves and authorizes the
Agreement between the Town of Queensbury and Wellchoice, Inc., to Provide Administrative
Services for the Retiree Drug Subsidy substantially in the form presented at this meeting, and
BE IT FURTHER,
RESOLVED, that the Town Board further authorizes and directs the Town Supervisor to
execute such Agreement and any other needed documentation and the Town Supervisor and/or
Town Budget Officer to take such other and further action necessary to effectuate the terms of this
Resolution.
