RESOLUTION AUTHORIZING USE OF HEALTH INSURANCE WAIVER FORM
RESOLUTION NO. 266, 92
, Introduced by Mr. Pliney Tucker who moved
for it's adoption, seconded by Mr. Nick Caimano.
RESOLVED, that the Town Board of the Town of Queensbury hereby
indicates it's approval of the use of the Health Insurance Waiver
Form that was previously presented to the Board Members as modified
at this meeting and also the questionnaire on Information On
Comparable Health Insurance Coverage.
Duly adopted this 4th day of May, 1992, by the following vote:
AYES: Mr. Caimano, Mr. Tucker, Mrs. Monahan, Mrs. Goetz,
Mr. Brandt
