This Agreement will serve as acknowledgement that Onsite Equipment, Inc. (Onsite) will
provide service on the Clarus Environmental, Inc. (Clarus) Fusion enhanced treatment
unit at the above referenced property. Onsite will perform annual scheduled service
checks of the Fusion for four years or until the first pump out is required. After this term,
the owner must subscribe to the inspection and maintenance program. Applicable
regulatory entity may be notified if the agreement is terminated at any time, regardless of
the reason.
Onsite will check the Fusion for proper operation and notify the owner of any deficiencies
that need to be addressed. The cost associated with additional items including
replacement of parts, pumping out accumulated foam/sludge or site grading is not
included in this agreement. Property owner agrees to operate the unit in accordance with
the manufactures recommendations as presented in the Fusion Owner's Manual. Owner
may transfer the benefits of this Agreement to any purchaser or other person who
acquires the property where the fusion is located.
Onsite is not responsible to redo any service checks delayed or prevented as a result of
limited access to the site, no electricity at the Fusion panel, or as a result of an Act of
God.
Property owner agrees to operate the unit in accordance with the manufactures
recommendations as presented in the Fusion Owner's Manual. The undersigned agrees
to the terms of this Inspection & Maintenance Agreement.

Dear Mr. Smith:
Congratulations on your recent purchase of the Fusion Treatment Unit as part of the septic
system at your house. This equipment is covered by a 10 year warrantee from the manufacture
and comes with four years of scheduled annual service checks by Onsite Equipment, Inc. I have
attached a Service Agreement for you to keep with your records. The Town Code Enforcement
office may ask for a copy of this Agreement, if they do please sign and provide them with a copy.
Onsite does not need a signed copy.
During the service visits, Onsite will take off the lids of the Fusion to visually see the inside of
the tank. We will adjust the settings, as needed, to achieve the recommended recirculation and
backflow rates. We will notify you if any additional work needs to be completed or if the Fusion
needs to be pumped out.
After the service term that comes with the unit expires, Onsite will offer a service program you
can subscribe to. This service is recommended by the manufacture of the Fusion and is required
in most jurisdictions. The current discounted rate for this subscription in your area is $195/year.
This price has been discounted for owners who have maintained an uninterrupted service
agreement with Onsite for the Fusion since the treatment system was installed. This rate for
service is also discounted for having the tank lids uncovered and accessible at the surface.
Lastly, a discount for having received payment within 30 days has been applied. Please do not
hesitate to call if you have any questions regarding your new Fusion Treatment System.
