Our office prepared a wastewater system design for James Mackey at the above-referenced
property. The system consists of a 4-module Puraflo Peat Biofilter treatment system, a 1,250-
gallon precast concrete septic tank, and a 1,250-gallon precast concrete pump station.
A representative from our office visited the site during system installation. Based upon our site
inspections and our discussions with the installer, it is my opinion that, with the exception of
surface finishing and turf establishment, the system was installed in general conformance with
the design prepared by our office and the attached record drawing.
