To Whom It May Concern,
PM&A was contracted to perform a Post-Construction As-Built Inspection on behalf of DISH Wireless at the
aforementioned cell tower site. Our engineer confirmed that the proposed DISH Wireless installation was
constructed and installed in general conformance with the Construction Documents prepared by PM&A,Revision 0,
Dated December 18,2018 and approved for permit by the Warren County.
Our engineer noted the following deviations from the approved Construction Documents:
1. Satellite dish was not installed per drawings. The elimination of the satellite dish was likely due to a
conflict with the satellite line-of-sight. DISH CM installed LTE solution antenna as alternative. These
field adjustments may be considered acceptable.
2. At the tower top level,the proposed DISH antenna mounts shown in drawings were not installed. DISH
CM and tower owner agreed to re-use the existing low profile antenna mount.The tower owner(Crown
Castle)has provided a passing Mount Analysis,dated March 17,2020 for the DISH installation and these
field adjustments may be considered acceptable.
All inspections are based on visual observations at the site. PM&A personnel did not perform a tower climb. All
tower top inspections were made with high powered binoculars,cameras and pictures provided by the contractor at
the time of construction. Additionally,PM&A did not perform any network RF performance testing or
electricaUoptical testing or grounding system evaluations.
