The application for the proposed system was approved by your office on May 16,2005. The approved application included design plans prepared by the office of The Environmental Design Partnership,stamped and signed by Dennis MacElroy,PE. These plans consisted of the following: • Sheet 1 of 2 SITE PLAN WITH PROPOSED REPLACEMENT WASTEWATER SYSTEM - Revision Date 5-6-05 • Sheet 2 of 2 DESIGN NOTES AND DETAILS Revision Date 5-6-05 As detailed on the design plans the system consists of an ejector pump assembly located in the house crawl space area with a 2"force main discharging to an upgradiant septic tank containing an effluent filter. Effluent from the septic tank flows by gravity to a two trench absorption field implementing a total of 56 feet of Eljen In-Drain(Type `B')units. Due to the schedule of the homeowner and contractor,as well as the fact that the existing system was continually operable until the final plumbing change-over,the actual installation took place over an approximate two month period. During that time I was able to visit the site on several occasions to observe the installation. It is my opinion that the material used and the installation performed was in compliance with the intent of the design plan.
