RESOLUTION AUTHORIZING MODIFIED WAIVER OF
HEALTH BENEFITS COVERAGE FORM

WHO MOVED ITS ADOPTION
SECONDED BY: Mrs. Connie Goedert
WHEREAS, the Town Board of the Town of Queensbury, by previous
resolution no. 266, 92, authorized the use of a health insurance
waiver form to be signed by employees who waive health insurance
benefits, and
WHEREAS, a modified waiver of health benefits coverage form
has been submitted to this meeting, said waiver in a form
acceptable to Town Counsel,
NOW, THEREFORE, BE IT
RESOLVED, that the Town Board of the Town of Queensbury hereby
authorizes the waiver of health benefits coverage form submitted to
this meeting.
Duly adopted this 1st day of July, 1996, by the following
vote:
AYES: Mr. Turner, Mrs. Goedert, Mrs. Pulver, Mrs. Monahan, Mr.
Champagne
