RESOLUTION AUTHORIZING AGREEMENTS BETWEEN THE TOWN OF QUEENSBURY
AND COMMUNITY HEALTH PLAN, MVP, EMPIRE BLUE CROSS/BLUE SHIELD,
AND NORTHCARE PARTNERS HEALTH PLAN

WHO MOVED ITS ADOPTION

WHEREAS, the Town Board of the Town of Queensbury desires to
offer health insurance to the Town of Queensbury's employees and
their eligible dependents, and
WHEREAS, the Town Comptroller's Office has reviewed many
health insurance proposals and has determined that the HMO plans
from Community Health Plan (CHP), MVP, Northcare Partners Health
Plan and Empire Blue Cross/Blue Shield Blue Choice, and the Empire
Blue Cross/Blue Shield Matrix Plan will best meet the health
insurance needs of the Town employees and has therefore recommended
Town participation in these programs,
NOW, THEREFORE, BE IT
RESOLVED, that the Town Board of the Town of Queensbury hereby
authorizes and directs the Town Supervisor to execute the remittal
agreements necessary between the Town of Queensbury and Community
Health Plan (CHP), MVP, Northcare Partners Health Plan and Empire
Blue Cross/Blue Shield Blue Choice, and the Empire Blue Cross/Blue
Shield Matrix Plan for the year beginning January 1, 1997, and
BE IT FURTHER,
RESOLVED, that said agreements shall be in a form to be
approved by Town Counsel.
Duly adopted this 18th day of November, 1996, by the following
vote:
