intennems cremations or other disposition of the body d a deceased person are made shall inter o permit the

in this aside.
z (a) The funeral dimcmr or undertaker shall deliver the burial perm@to the person in charge of the place of burial or
other disposition before interring or dherwise disposing of the body or shall attach the removal or transit permit to the
box containing the hotly,when .shipped by any transportation company,which permit shall accompany Me remains to its
destination, where, B within this state, it shall be delivered to fie person in charge of the place of bunal or other
disposition
(hl Any person orotherentityowning operating managing adesignatedtoreaive the body da deceased person at a
place of burial. cremation, or other final disposition In this state, who receHes the body of a deceased person, shall
provide a receipt for the body to the funeral director. undertaker or registered resident who delivered such body. Each
receipt shall (I) he endorsed by hath such person and the funeral director, undertaker or regulated resident Oil indicate

charge of the place of burial, cremaion or other final tlispmition for a period of not less than far years, and shall be
made available for inspection by the division dcemeteries during normal business hours The original copy ofevery such
remipt shall be retained by the licensed funeral finn for a period of not less than four years pursuant to the rules and
regulations of the department governing the maintenance d records.

d istrid within seven days after the date of i nterment cre mation cr other disposition.
