RESOLUTION TO APPOINT REGISTRAR OF VITAL STATISTICS
AND RECORDS ACCESS OFFICER
RESOLUTION NO.: 12, 1999

WHEREAS, in accordance with §4121, Subdivision 3(A) of the New York State Public
Health Law, the Town Board of the Town of Queensbury wishes to re-appoint Darleen Dougher as
the Registrar of Vital Statistics and Records Access Officer,
NOW, THEREFORE, BE IT
RESOLVED, that the Town Board of the Town of Queensbury hereby re-appoints Darleen
Dougher as Registrar of Vital Statistics and Records Access Officer to serve at the pleasure of the
Board, and
BE IT FURTHER
RESOLVED, that immediately upon Ms. Dougher’s acceptance of appointment, she shall
appoint a Deputy to act in her place and stead in case of absence or inability and the Deputy shall
accept this appointment in writing.
